ROM PAD AREA

  • Are road ways graded and all road signs clean and placed where they can easily be seen

  • Light Vehicle access road is clearly separated from the truck haul road?

  • Is Light Vehicle parking areas clearly demarcated with reverse parking stops in position? Is the area being used for Light Vehicle Parking?

  • The area around the Crusher Dump Pocket is clear of rocks and rubble that could cause a tyre puncture or equipment damage? <br><br>Report any identified hazards to the ROM Pad Supervisor

  • Is the Primary Crusher access doorway and crane bay clear of equipment, rocks and rubble, mud and materials that could cause a slip, trip or fall for personnel working in the area?

  • Is all of the ROM Pad lighting at the Primary Crusher area working correctly and are the lens covers clean and bright?

  • Can the truck dump lights be seen easily from the haul trucks and are they clean and operating correctly?

  • Is rubbish (i.e. Wood, paper, plastics, welding electrodes, paper cups, plastic bottles etc) placed in a rubbish bin that is painted green for general waste?

  • Is Scrap Metal and Steel waste been placed into a Blue Scrap Metal Bin?

  • Can Fire Extinguishers be clearly seen or identified and are they stored correctly? (i.e. Fire Extinguisher either hung on a wall and clear signage or painted location).

  • Are Fire Hydrants clearly demarcated with hoses rolled up and all attachments available for operation in an emergency?

  • Are toilets available for Operators and Maintenance in this area?

PRIMARY CRUSHER BUILDING - Crane Bay Area

  • Is the control room area clean and clear walkway access with no slip, trip or fall hazards present?

  • Is water drainage in the crusher crane bay working with no water pooling and/or mud pools on the concrete?

  • Are all ladders, stairways with hand rails and walkways in good working condition with no hazards or obstructions that can cause a slip, trip or fall?

  • Are all lights working in the crane bay and are the lens covers clean?

  • Is the Overhead Bridge Crane left parked over the crane bay with the hooks in the raised position and is the remote control available for the crane.

  • Does the Overhead Bridge Crane have the direction of travel clearly marked North, South, East and West so the operator can see this. Does the remote control for the crane have the North, South, East and West marked on the control buttons and does this match the direction of travel shown on the crane?

  • Are the up and down buttons clearly marked on the remote control for the Overhead Bridge Crane - for both hooks and does this match the actual operation of the crane?

  • Are all water or air hoses rolled up and stored correctly away from walkways and work areas?

  • Have you observed any personnel working in this area and are they wearing all of the required PPE and is it being used correctly?

PRIMARY CRUSHER - Lubrication System Floor

  • Are all ladders, stairways and walkways clear of debris or spillage and there are no rubbish or materials causing slip, trip or fall hazards?<br>If the answer is no then please identify in comments and contact the responsible supervisor to have it fixed.

PRIMARY CRUSHER - MCC Room and Drive Train Floor

  • Are all ladders, stairways and walkways clear of debris or spillage and there are no rubbish or materials causing slip, trip or fall hazards?<br>If the answer is no then please identify in comments and contact the responsible supervisor to have it fixed.

  • Is a standard Operating Procedure (SOP) used for the works that in progress?<br><br>If the answer is no then please identify in comments.

  • Are the personnel working on the job aware of the actual hazards and/or risks with the task or tasks?

WORKER OBSERVATIONS - General Tools and Personal Hand Tools

  • Electric hand tools in use are tagged by electrician showing quarterly earth leakage testing has been completed?<br><br>If the answer is no then please identify in comments and contact the responsible supervisor to have it inspected, tested and tagged. The equipment should be placed "Out of Service" until this is completed.

  • Is all lifting and rigging equipment carrying the current quarterly inspection tag and is it current and up to date?<br><br>If the answer is no then please identify in comments and contact the responsible supervisor to have it inspected, tested and tagged. The equipment should be placed "Out of Service" until this is completed.

  • The workers have their personal hand tools with them and they are stored in a tool bag or tool box. The condition of the tools is good and clean?<br><br>If the answer is no then please identify in comments.

  • Welding equipment including welding leads, welding hand piece and the earth lead are in a good condition with no insulation breaks or wire exposed. Is this equipment being used correctly?<br><br>If the answer is no then please identify in comments.
