Are supervisors in charge of work groups that are required to undertake Hot work operations trained in the SLAM and or JHA methods and received training in the specific hazards, risks and controls that apply to hot work and given the responsibility by the Engineering manager of Authorized Hot Work permit issuer ?

As a minimum SLAM is completed for all Hot work tasks?

Is the HWA complete the Hot work permit form and address the questions presented on that form and identify the necessary steps to eliminate or control the hazards identified and documenting on the hot work permit form

Is the Hit work permit form presented to WPI in charge for review and approval?

Where the risk assessment is high with controls in place then a full JHA is undertaken?

Is the HWA implement the risk control measures listed on the Hot work permit before commencing the Hot work?

On completion of the Hot work, is the HWA satisfies himself that the conditions on the hot work permit have been satisfied including necessary fire watch and clean up and making safe of the job site after which he can sign off the hot work as complete on the hot work permit and take the permit to WPI for sign off

Is the WPI satisfied himself or herself that the work location and vicinity are left safe and no further residual risk of fire or ignition exists before signing off closure of the hot work permit
