  • Have all operatives on site signed the task specific RAMS before starting work?

  • Have all operatives signed in to site by either the Donseed thumb print device or signing in sheets?

  • Has the supervisor / Project Manager attended a client led co-operation / co-ordination meeting in the last month?

First Aid Facilities

  • Are first aid facilities to be inspected in this audit?

Fire Prevention & Emergency

Site Security

Trip and Fall Safety

  • Are cables, leads and hoses routed safely and protected against slips, trips and falls?

  • Are pathed corridors and stairs well located and free from slipping and tripping hazards?

  • Are railings and footpaths in good condition?

  • Is all work above 2m, or within 2m of an edge being done safely?

  • Are all openings and excavations protected from a fall from height with either heras fencing, barriers, suitable covers or a combination of these?

Manual Handling of Materials and Equipment

  • Hazardous Manual Tasks to be inspected in this audit?

  • Is equipment such as excavators and dumpers being used to move loads / materials as close to the work area as possible?

  • Are operatives lifting materials and equipment within their individual capabilities and using correct manual handling techniques?

  • Are kerb lifters or similar equipment being used for lifting kerbs into position?

General Machinery, Plants and Equipment

  • General Machinery, Plants and Equipment to be inspected in this audit?

  • Are relevant pre-start inspections being completed on machinery, equipment and mobile plant with daily check sheets completed?

  • Did all mobile plant seen during the audit seem in good order i.e. not faulty or damaged? If not, has the fault / damage been reported to the supervisor and plant department?

  • Do all excavators / telehandlers have a thorough examination certificate issued within the last 12 months and is a hard or electronic copy available on site?

  • Do all mobile plant operators hold the relevant qualification such as CPCS or NPORS cards for the plant they are operating?

  • Are seat belts being worn correctly on mobile plant including: Rollers, Dumpers, Excavator's, Telehandlers or any other mobile plant?

  • Are sthil saws being used with water suppression? and are operators trained / competent, face fit tested and wearing eye and ear protection and dust masks?

  • Are all vibrating hand tools appropriately managed to highlight daily exposures for vibration where required in accordance with the Vibration risk assessments?

  • Are all excavator buckets safely stored and adequately fenced off and have appropriate signage?

  • Have chains, slings and other lifting accessories been inspected within the last 6 months, in good condition and are the certs on site, either hard copy or electronically?

  • Are dumpers being driven safely? such as not driving on heaps, not speeding, not sitting on seat belts and not wearing hoodies?

  • Are mixers positioned in the relevant locations on site and do Mixers have adequate blocks and vehicle protection to protect the mixer operator?

  • Is adequate fencing and / or barriers in place to segregate vehicles and pedestrians?

Breaking Ground

  • Is ground being broken or opened up?

  • Is a permit to dig in place?

  • Has the area been CAT Scanned?

  • Are CAT scan operatives trained and competent?

  • Are trial holes dug by hand to ascertain the line and depths of services?

  • Are excavations being dug by hand when within 500mm of a known burried service?

  • Do excavations have adequate fencing / barriers around them to protect from a fall from height into the excavation?

  • Is suitable shoring in place for deep excavations (deeper than 2m) to prevent the trench from collapsing?

  • Are excavations deeper than 1.2m suitably stepped or battered back at 45 degrees?

Chemical Safety

  • Is all COSHH related issues checked in this audit?

  • Is a Hazardous Substance Register complete and are COSHH assessments available?

  • Are bulk fuel containers in good condition and set up on polythene and sand or shingle to act as a drip tray, with barriers / fencing around them to protect them? Is a fuel point sign in place?

  • Are wheelie bin type spill kits in place (one per fuel container) or other clean up arrangements available?

Confined Spaces

  • Confined spaces safety to be inspected during this audit?

  • Do all applicable confined space works have adequate permits granted and documented?

  • Is a confined space emergency plan in place?

  • Are gas monitors used prior to entry into the confined space?

  • Are confined spaces being continually monitored?

  • Are all workers undertaking confined space works trained in confined space entry and activities?

Environmental

  • Is the Environment to be inspected on this audit?

  • Is site waste segregated and disposed of correctly in designated skips?

  • Is nuisance noise, vibration and dust controlled on site?

  • Is surface water and run off water managed and controlled?

  • Are gullies and drains protected with gully bags or hay bails and are these cleaned out or replaced on a regular basis?

  • Are bulk fuel containers situated at least 10m form any water courses including manholes and gullies?

  • Does the site have any tree preservation orders, newt fences or any other areas of interest from the Environment Agency?

  • If Yes to the above, are these areas protected, fenced and is adequate signage displayed?
