Assessment Purpose

ASSESSMENT PURPOSE

  • Wing Safety office conducts an assessment of the unit's Mishap Prevention program to evaluate its compliance with regulatory requirements according to United States Law, applicable Department of Defense, Air Force and Air National Guard instruction.

    Assessment Overview: The goal of the assessment is to identify gaps in the unit Commander's Mishap Prevention Program that exposes the unit’s personnel and Commander to unnecessary risk. It should be used to provide unit leadership awareness to areas requiring additional resources and support from the Wing Safety office. This assessment is required by Public Law, DOD Requirements, and Air Force Instructions.

    The Wing Safety office will track all program deficiencies. Supervisors and USRs are responsible for providing status updates every 30 days. All identified deficiencies are in the following report.

TERMS & DEFINITIONS

  • DEFICIENCY: A validated deficiency assessed as CRITICAL, SIGNIFICANT, or MINOR:

    - CRITICAL: Any deficiency that results or could result in widespread negative mission impact or failure.

    - SIGNIFICANT: A validated deficiency that has or could have negative mission impact.

    - MINOR: A validated deficiency that does not meet the definition of a Critical or Significant Deficiency but requires corrective action.

    OBSERVATION: General statement made by inspector regarding a process, inspection item or area of interest.

    RECOMMENDED IMPROVEMENT AREA (RIA): An identified process, product, or capability which could be improved by a suggested course of action.

GRADING

  • Possible assessment grades are:

    MET AND EFFECTIVE: All program elements of the flight safety program conformance and effectiveness under the Air Force Safety Management System (AFSMS) were found to be effective.

    EFFECTIVE BUT NEEDS MINOR IMPROVEMENTS: Most program elements of the flight safety program conformance and effectiveness under the Air Force Safety Management System (AFSMS) were found to be effective, with some needing only minor improvements.

    EFFECTIVE BUT NEEDS SIGNIFICANT IMPROVEMENTS: A large number of program elements or a key component of the flight safety program conformance and effectiveness under the Air Force Safety Management System (AFSMS) are in need of improvement, but the program was overall effective.

    NOT EFFECTIVE: A large number of program elements or a key component of the flight safety program conformance and effectiveness under the Air Force Safety Management System (AFSMS) was found to be ineffective.
