Instructions

  • 1. Each question contain 10 marks.

  • 2. Observation for each question of major category will deduct 3 marks.

  • 3. Observation for each minor category will deduct 1.5 marks.

  • 4. Multiple observations for each question of major category will deduct 3 marks per observation.

  • 5. Multiple observations for each question of minor category will deduct 1.5 marks per observation.

  • 6. If the deduction is more than or equal to 10 marks, the final score awarded will be zero.

  • 7. If no observation is found w.r.t. the question, the awarded marks will be 10.

  • 8. Weightage for minor category is 10 marks while weightage for major category is 20 marks or more depending upon the importance of the question.

  • 9. Attach pictures and add notes for each observation.

SORT

Unneeded Items

  • Is there no unneeded item/s present in the working area?

  • Can all unnecessary items be distinguished at a glance? (Placed in a specific area/Red Tagged Area)

Needed Items

  • Needed items have been placed at the closest location to minimize the motion's waste.

Backlog

  • Are there any items present in the area which cannot be identified? Or they belong to any other department?

  • Are the 'backlog' item sorted from 'completed' items?

SET IN ORDER

Indicators & Demarcations

  • Are shelves, stations, bins, trolleys, apparatus, and other items etc. labelled in good condition? (Labeling should not be worn out, clearly visible, and as per the standard format)

  • Are walkways, aisles, storage areas, backlog areas, WIP areas, FG areas and scrap areas clearly demarcated in good condition?

  • Are wall plates installed at appropraite locations and clearly visible?

Placement

  • There is designated place for parking equipments like trolleys, ladders, dustbins, Lifters

  • Equipment and items are placed as per their labeling and demarcation

  • Are there no items/equipment/tools/apparatus in the working area that have not been put away immediately after their use? (minor)

Safety Hazards

  • Are there no safety hazards (water/oil/chemicals/open electrical wiring) in the working area?

  • Are the safety equipment and supplies in clear and obstacle free?

Sign Boards & Visual Controls

  • Are the visual controls and indicators established in the working area? Like station lights, Entry/Exit signs

SHINE

Floor and other Surfaces

  • Are the floor, walls, stairs and, all other surfaces in the working area free of dirt, oil and grease?

  • Is there any garbage on the floor, desk or anywhere within the premises of the working area?

Cleaning Responsibility

  • Is the person responsible for overseeing cleaning operations present in the premises of the department?

Cleaning Material Availability

  • Are cleaning tools and equipment available and adapted?

STANDARDIZE

  • Is the Cleaning Checklist present on the notice board and filled daily? (minor)

Layout

  • Are the entry and exits of the department obstacle-free? (minor)

Understanding of 5S

  • Can TL, manager and other staff explain what is 5S and why 5S is important?

Standards Control

  • The color coding used for demarcation is as per daraz standards?

  • Same labeling style (font, size , color) is used at all locations (inc wall plates)

  • Station officers are using the correct station light protcols as defined per daraz standards

SUSTAIN

Training

  • Is the staff trained about the standard procedures of 5S protocols?

Follow-up and Reporting

  • Is the 5S Genba Sheet Filled daily by the 5S champion before the cut-off time(3PM) ?

  • Are the 5S Champions being selected and displayed in the designated working area?

  • Are all the 5S issues being highlighted/recorded with resolution timelines?

Suggestions & Improvement Ideas

  • Does the team adhere to the suggestions and improvements provided by the 5S audit team?
