Probation Review and GMH

  • Date of Probation Review? Put in notes. Ensure the employee understands that at any time during their probation extension if they fail to achieve the required expectations their employment may be terminated on the grounds of 'unsatisfactory completion of probation period'.

  • Has the employee got a copy of this form? If not, please issue them with one, then file a copy in their personnel file.

  • If a probationary period is extended, it is recommended that it is for no longer then 4-6 weeks. Specific objectives must be based on the areas of "needs improvement" monitored and reviewed throughout the extension. The objectives should be recorded on this form. Ensure the employee understands that at anytime during the extended probationary period that if they fail to achieve the required agreed expectations their employment may be terminated on ground of "unsatisfactory completion of their probationary period". All completed forms should be filed within employees personnel file. A photocopy of this form should be given to the employee. Hourly paid employees: if guaranteed minimum hours (GMH) have not been offered previously they should be offered on successful completion of the probationary period. Follow the normal procedures for GMH; and log the hours on ResourceLink. Please refer to the guidance materials and issue a GMH contract with continuous service if applicable.
