Documentation

  • Are current and most updated documents,procedures, and policies being utilized?

  • Are any changes to drawings, manuals, procedures, and wiring diagrams being noted, documented in red, and submitted to Houston for review?

  • Are the PM checklists/manufacture drawings being utilized for all equipment prior to use in the field?

  • Are the checklists provided filled out in its entirety?

Identification and Traceability

  • Where applicable are assemblies being identified by markings/labels to maintain traceability?

  • Do serial/asset numbers match the PM checklists?

  • Discrepant material had been clearly marked/ identified and whenever possible, segregated to prevent unintentional use?

  • Chemicals and other shelf life items (I.e. O-rings) in use and in consumables cabinet have not exceeded expiration or use by date(UBD) Labels are legible.

Packaging, Handling, Transportation and ESD

  • At a minimum, ESD safe workstations are configured in accordance with QCD 15.1 - ESD stations are identified with a sign or poster.

  • Are O-rings/seals stored properly and within manufacture's recommendation ( I.e. black bag with exp. date displayed)

  • ESD safe workstations are clean , neat and free of unnecessary static generating material ( plastics, paper, foam, drink bottles, etc.)

  • Electronics personnel using proper ESD lab coat.

  • ESD sensitive components and assemblies containing exposed PCBAs or ESD sensitive components must be stored or transported in sealed static shielded bags/ boxes. Exception: Tools left on employee's ESD safe workstation are exempt.

  • Racks and carts used to store/transport ESD sensitive components/assemblies are grounded with a chain that makes contact with an ESD conductive floor surface. All shelves of metal racks must be at a common potential - no isolators between shelving.

  • All personnel handling ESD sensitive components / assemblies must only do so at ESD safe workstation using a functional / tested grounding system.

  • Conductive shoes are tested prior to commencing work ? Urn monitored wrist straps are tested prior to handling product. A daily log sheet is maintained.

  • Completed assemblies are packaged as to prevent physical damage ( I.e. connector caps, bubble wrap, etc.)

  • Machine assemblies with threads are protected with required designated thread protectors during all storage and transportation. ( mechanical and electrical)

Calibration

  • Equipment used to assemble , test evaluate and accept products has not exceeded the calibration due date - NOTE: the calibration past due date will be the end of the month ( ex: due 9/4/08 is past due on 10/ 01/08.)<br>Calibration labels are legible and not marked over.

Competency Training

  • Personnel and understand and know where to locate the Aker Solutions Well Service Operating System?

  • Personnel can demonstrate how to find a current procedure

  • Employees know where their training plan is located and understand the purpose?

  • Training had been identified and scheduled for each employee

  • Training plans are reviewed and updated on a quarterly basis

  • Training plan is posted for employees use.
