Is the site First Aid Kit present and stocked?

Are designated First Aid providers on-site?

Is a minimum 5-pound fire extinguisher provided and tagged?

Is the Emergency Response Plan established and posted (call 911 and Excavation Staging Area set-up)? Does the crew know the project address and utility shut-off locations?

Is Personal Protective Equipment available and being worn by all personnel (gloves, ear protection, safety glasses, hard hats, and dust masks?

Are ladders provided and free of structural defects?

Are ladders used properly and secured when needed?

Are electrical chords free of cuts and kept out of wet areas?

Are electrical chords run free of tripping hazards (especially down stairs)?

Is the generator maintained; grounded if multi-phase and > 5,000 Watts? (Generators over 5,000 Watts can be grounded to vehicle frame).

Are electrical outlets and panels covered and free of fire hazards? Is wiring grounded? Is lockout/blockout respected?

Are scaffolds set-up properly? Platform stock plumb and level?

Are mud sill plates nailed to base plates; scaffold tied off every 15 feet?

Are top, mid and end guardrails secured and erected at 7 1/2 " or taller?

Are planks 2 x 10 crack free and cut free and overlapped 6" to 8"?

Is scaffold tied-off with-in 16" of building?

Are the scaffold plank span distances 10 feet (or 7 feet if heavy use of 75 p.s.f.)?

Are scaffold toe boards erected at points above where people enter the building or work area below?

Is the attic and/or basement of the house safe for workers?

Are excavation areas fenced, checked for exiting utilities; protective systems (i.e., shoring) used when required due to soil type, work done, personnel in excavated area, entrance ladder, checked foot new hazardous conditions - rain or nearby grading (dozer) vibrations?

Is brush cleared 100 feet from jobsite; No Smoking posted in high fire areas (unless area is designated safe with ashtrays provided)?

Is the jobsite being kept clean at all times, averting accidents?

Is potable water available on-site?
