RISK ASSESSMENT AREAS

DIG ALERT

  • Is Dig Alert ticket number written on work order

  • Did foreman verify with Lease Agent or Project Manager that the Dig Alert has been completed

  • Did foreman verify utility markings on project prior to starting work

  • If digging/pounding is within 24" of marked lines, did crew hand dig/pound

TOOLS

  • Are hand and power tools in good condition (no broken handles, missing grounding prong, missing guard)

  • Is portable generator in good condition. GFI working properly (when tested the GFI works properly)

  • Are heavy duty rated extension cords in use, in good condition with grounding prong in place, and no splices or insulation breakage

  • Are compressor and air hoses in good condition. Proper air hose connections are being used and no spliced air lines

MATERIAL HANDLING

  • Workers use proper lifting techniques when unloading materials (slides materials off of truck or trailer using weight of material to carry it off of vehicle instead of lifting entire load up)

  • When possible did crew use two man lifting technique

  • When possible did crew use equipment to carry materials (forklift, wheelbarrow) or move truck closer to work area to reduce the load carrying distance

  • Did workers use proper lifting techniques (worker bent at knees) and carrying techniques (load close to body)

PUBLIC PROTECTION

  • Post holes are not left unmarked or open

  • When vehicle is parked on or along roadway, are traffic cones properly positioned to alert/deter public traffic (starting at 150')

  • Vehicles and equipment are parked in safe location so not to create risk to pedestrians and traffic

  • A spotter is used when backing up the vehicle

LOAD SECUREMENT

  • Minimum of 3 load securement devices per 20' length of material being secured (if back stop is used 2 load securement devices are ok)

  • Load securement devices are in proper condition (not to have a combined total of over a 1/2" in damage the entire length of securement device)

  • All small items (bolts, hinges, wheels, etc.) are in containers (buckets) and not laying in bed of truck

  • At least 1 spare load securement device on vehicle

  • Material/Panels on vehice/trailer are stacked straight and orderly.

VEHICLES

  • Daily Vehicle Inspection Report (DVIR) completed and in truck.

  • Truck and Trailer have current registration documentation

  • Current proof of insurance documentation in vehicle

  • Current Annual DOT documentation for truck and trailer in vehicle

  • All tires on vehicles are in good condition

  • General condition/appearance - lights working, clean, no new dents

  • Pintle hook, donut ring, and emergency pull system working properly (hook or ring not showing excessive wear, battery charged on pull system)

  • Conspicuity tape in good condition and covering at least 50% of bed

  • Walking / working surfaces in good condition (no holes or exposed metal edges)

  • Workers use 3 points of when climbing on or off of vehicles

PERSONAL PROTECTION EQUIPMENT (PPE)

  • Are workers wearing the required Personal Protective Equipment (Long pants, at least 3" long sleeves, Hi-visibility vest if working along roadway or around heavy equipment, hard hat if working with items above the head, hearing protection if compressor or generator are operating, appropriate gloves, steel toed shoes, safety glasses)
