WALKING-WORKING SURFACES

Stairways and Railways

  • Stair treads have a reasonable slip-resistant construction with nosings made similar.

  • Standard railings are on the open sides of all exposed stairways and stairwells.

  • Handrails are on at least one side of a closed railway.

  • Flights of stairs with four or more risers are equipped with standard handrails.

EGRESS

Routing Obstructions

  • Route must be adequately lighted so that an employee with normal vision can see along the exit route.

  • Exit must be clearly visible and marked by a sign reading "Exit."

  • Exit route door must be free of decorations or signs that obscure the visibility of the exit route door.

  • Signs must be posted along the exit access indicating the direction of travel to the nearest exit and exit discharge with the line-of-sight to an exit sign clearly visible at all times.

  • Each doorway or passage along an exit access that could be mistaken for and exit must be marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use.

  • Exit signs must be illuminated by a reliable light source and be distinctive in color.

  • Each exit sign must have the word "Exit" in plainly legible letters not less than six inches high, with the principle strokes of the letters in the word "Exit" not less than three-fourths of an inch wide.

Alarm System

  • Every AMR facility shall have an effective employee alarm system in accordance with the National AMR Emergency Action Policy.

  • The employer has established procedures for sounding emergency alarms in the workplace.

  • The employer shall assure that a test of the reliability and adequacy of non-supervised employee alarm systems is am every two months.

Emergency Action Plans

  • Procedures for reporting fires or other emergencies are in place.

  • Procedures for emergency evacuation, including type of evacuation and exit route assignments are in place.

  • Procedures to account for all employees after evacuation are in place.

  • Facility practices and documents evacuation drills at least annually.

OCCUPATIONAL HEALTH AND GENERAL ENVIRONMENTAL CONTROL

Lockout/Tag out (lLOTO)

  • LOTO program serviced by outside contractor.

  • Operations/Fleet has Lock Out/Tag Out program for control of hazardous energy.

  • An authorized person has been identified for servicing and maintenance of equipment in LOTO program.

  • Annual inspection of LOTO program conducted and documented.

  • Each authorized and affected personnel are trained as to the hazards and purpose of the control program.

HAZARDOUS MATERIALS

Compressed Gases

  • Transfixing stations, or Cascade Systems have been dismantled.

  • Compressed gas storage areas are protected from being struck and are in secured area. Tanks are secured from falling.

  • Compressed gases are stored at least 30 feet from heat sources.

  • Compressed storage areas shall have appropriate signage in accordance with AMR National Compressed Gas and Fire Protection Standards.

  • Safety caps must be in place on all tanks that are designed to accept safety caps.

Personal Protective Equipment

  • Unauthorized PPE are not found at the operations.

FIRE PROTECTION

Fire Extinguishers/Suppression - General

  • Portable fire extinguishers are mounted and located so that they are readily accessible to employees without subjecting the employees to possible injury.

  • Portable fire extinguishers are maintained in a fully charged and operable condition and kept in their designated places at all times except during use.

  • Fire suppression systems (sprinklers) are clear of obstructions in accordance with federal standards.

Inspection, Maintenance and Testing

  • Proof of inspection, maintenance and testing of all portable fire extinguishers in the workplace is documented.

  • Proof of monthly visual inspections of all portable extinguishers is evidenced.

  • Alternate equivalent fire protection is provided when portable fire extinguishers are removed from service for maintenance and recharging.

  • Smoke alarms shall be installed in every sleeping area, in the adjoining hallway and near kitchen areas.

  • Smoke detectors should be tested at least monthly.

Storage of Flammable/Combustible Materials

  • All solvents, degreasers or shop chemicals that are labeled as flammable or combustible shall be stored in a clearly labeled flammable materials cabinet when not in use.

  • All used solvent or oil soiled rags are properly contained in an approved flammable materials waste container.

  • Chemicals with different hazard classifications are stored separately.

MACHINERY AND MACHINE GUARDING

Machine guarding

  • Guards affixed to machines where possible and secured elsewhere if for any reason attachment to the machine is not possible.

Point of Operation

  • Exposure of blades. When the periphery of blades of a fan is less than seven (7) feet above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch.

  • Machines designed for a fixed location shall be securely anchored to prevent walking or moving.

  • Abrasive wheels shall be used only on machines provided with safety guards. The safety guard shall cover the spindle end, nut and flange projections.

  • Work rests shall be of rigid construction and designed to be adjustable to compensate for wheel wear and shall be kept adjusted with a maximum opening of one-eighth (1/8) inch to prevent the work from being jammed between the wheel and the rest. The work rest shall be securely clamped aft each adjustment . The adjustment shall not be made with the wheel in motion.

WELDING, CUTTING AND BRAZING

Cylinders and Containers

  • Compressed gas cylinders shall be legibly marked with either the chemical or the trade name of the gas. Such marking shall be by means of stenciling, stamping, or labeling, and shall not be readily removable.

Cylinder Storage

  • Acetylene cylinders shall be stored valve end up.

  • Cylinders shall be kept away from radiators and other sources of heat.

  • Empty cylinders shall have their valves closed.

  • Compressed gas cylinders shall be secured in accordance with AMR Compressed Gas Standard.

ELECTRICAL

Working Space

  • Flexible cords and cables (extension cords) are only used as temporary wiring in accordance with 29 CFR 1910.395(g)(1) or state standard equivalency and AMR National Fire Protection Standard.

  • Portable space heaters have automatic shut-off feature. Portable space heaters are only plugged into permanent wiring.

Electrical Service Panels

  • Flammable and combustible chemicals shall be kept at least thirty (30) feet from all electrical panels and sub-transformers.

  • The width of working space in front of the electrical service panels shall be the width of the equipment or 30 inches, whichever is greater. The working space shall permit at least a 90-degree opening of equipment doors or hinged panels.
