1. Availability of Literature of Guide Prescribing

  • Prescence of EML and STGs- Hard copies and applications on cell phones.

2. Prescriptions

  • Patient details are on the script

  • Diagnosis corresponds with medication prescribed

  • STG's/EML are adhered to.

  • Dosing of medicines, frequency, and route of administration is appropriate for patients age, weight, body surface are and gender.

  • Date, time, and signature for dose administered by nurse.

3.Clinical Notes

  • All medication doses, interactions and allergies and document changes in the patients file.

  • Recording of special requirements (nasogastric Tubes. TPN's etc.)

  • Communicate with the prescriber regarding special requirements.

  • Review laboratory results for dosage requirements

4. Stock Availability

  • Stock availability of medication at wards

  • Schedule 5/6 and emergency checks

5. Prescription Queries

  • Follow up on the prescription queries with prescriber. Example: Switching of patient on IV medicines for more than 3 days to oral medicines.)
