  • This CHECKLIST has been customized to suit the inspections of ASDA Retail stores. The purpose of this CHECKLIST is for training/guides only. Please always continue to complete the correct report documentation as per agreement. This report can be used in conjunction with the F2533 form, and can be sent to the store GSM as a PDF format.

  • Before your Inspection, ensure you inform management/security that you are on site, and always sign in using the appropriate methods. Please ensure you check the accident book before starting your inspection, to ensure you're aware of previous dangers in the workplace. After the inspection, ensure that management is informed of the problems and takes appropriate action to resolve them. Always make your reports in writing using your GMB Safety Representatives Report Forms F2533.

  • SAFETY REPRESENTATIVES AND SAFETY COMMITTEES SRSC Regulation 5, (1-4) will provide you with your entitlement to complete Inspections of the workplace.

Checklist

CARPARK

  • Are all pedestrian routes clear from any trip hazards?

  • Has all fire doors got clear marked signage?

  • Are floor gullies clear of any damage, raised, or sunken areas?

  • Are all fire exit routes clear from all obstructions?

  • Are all trolley bays clear from any damage to the perspecs, or clear from any structural damages?

  • Are all lights working around the perimeter of the building, and around public access points to the store, not being blocked or obstructed by overhanging trees?

  • Is 10mph marked signage visible around the carpark?

  • Is the fire assembly point clearly marking within the carpark?

  • Are all curbing stones in good condition, not raised, or broken causing trip hazards?

  • If applicable are all surrounding walls/fences in good condition, with no visible damages?

  • Are all road markings visible, with no faded areas around the carpark?

  • Are all fire doors around the perimeter of the building securely closed?

  • Are the delivery gates leading into the warehouse, secure, and kept closed, with no access to public?

  • Are there any broken trolleys around the carpark, which will need to be removed?

  • Is the smoking shed in good condition, with no visible or known damages?

  • Are all customer/colleague benches, seating areas outside, safe, sturdy with no known damages?

  • Are all bushes, trees, shrubs well maintained around walkways, and parking bays?

  • Is there any known health & safety concerns from porters, is all PPE provided, and warn?

  • Does the store have sufficient amount of different trolleys accessible for General public, Parent & Baby's, Disabled?

  • Are all public entrance doors in working order, with no visible damage?

  • Are all carpark bollards in good condition & visible to the public?

  • Are all pedestrian footpaths protected by bollards?

  • If applicable, are all salt/grit boxes in the carpark, free from rubbish, only containing salt.

  • Are Click & Collect collection points in good condition, with no visible damage, trip hazards?

  • Is the Click & Collect drive-thru protected by CCTV & does this work?

  • Does the Click & Collect drive-thru have zero tolerance posters displayed?

  • Is there any other concerns found at the time of safety audit in the carpark?

Fuel station & Forecourts

  • Are all fuel forecourt lights working, illuminating the area well?

  • Is the fuel venting system clearly marked, with no visible damage to the system or surrounding wall?

  • Are all gloves/paper towels made available throughout the provided bin areas?

  • Is the PFS easily accessible for colleagues to walk to & from the store?

  • Is there any visible or known damage to the structure of the fuel forecourt canopy?

  • Are there any trip hazards around the fuel forecourt, including all floor gullies?

  • Are all fire extinguishers within service date, and sand buckets made available?

  • Is there a sufficient amount of fire extinguishers around the forecourt?

  • Are all the sand buckets only containing sand?

  • Are all grit boxes around the forecourt clear of all rubbish, and only containing grit?

  • Are locked/secured areas, securely locked at the time of inspection?

  • Are all air/water machines in good working order, with no visible or known damage?

  • If applicable is the car wash in good working order, with all glass surrounds clear from cracks?

  • If applicable, is the car wash clear from any visible or reported damages?

  • Are all pedestrian walkways clear, with no trip hazards?

  • Are all PFS bollards in good condition, and clearly painted?

  • If applicable, are all fridges working within the fuel station (Manned small fridges), including shop floor fridges/freezers?

  • At the time of inspection are all fridges/freezers free from any leaks if applicable in the FPS?

  • If applicable are all fire doors, clearly marked up with the correct signage, and secured?

  • Is the any health & safety concerns from colleagues within the PFS?

  • Are all first aid kits stocked correctly, all containing, eye wash, plasters, icepacks?

  • Are the colleagues provided with own PPE? This can include H-Viz, Safety Boots.

  • Does the colleagues wear the provided PPE?

  • Is there any other concerns found at the time of safety audit in the PFS?

Home Shopping/Click & Collect

  • Does the home shopping fridges/freezers have current fire alarm sirens/sprinkler system installed?

  • Are home shopping van bays in good condition, with no visible damage to the structure, the safety barriers, and safety wheel bumps?

  • Does the home shopping bays, have sufficient amount of lighting?

  • Are the home shopping vans in good condition, with no un-roadworthy vans being used?

  • Are all Home Shopping colleagues wearing the correct PPE for their job role?

  • Are sack barrows being stored in the front of the cab with the drivers?

  • Are Hi-Viz jackets/vests made available for click & collect staff

  • If applicable does all the van keys have the safety tool?

  • Are all Hi-Viz signage for entering areas where PPE is required, visible not faded?

  • Is PPE being adhered to & followed?

  • Is the first aid box clearly visible, correctly stocked?

  • Are all picking trolleys in good condition with no clear damage?

  • Any known injuries to pickers whilst using 10 tote picking trolleys?

  • Are all near misses/accidents being reported within the department?

  • Is there any PPE or equipment needed to be ordered or replaced?

  • Are there any issues when ordering PPE or Equipment?

  • Are all broken sack trucks being sent back to supplier, or disposed of correctly?

  • Are all Sack Barrows withing the department in good working order?

  • Are all electrical charging points for delivery vans in good condition, with no broken cables, or sockets?

  • Are all totes stacked correctly, to an ideal height?

  • Are all trolleys/totes/bases in good working order?

  • Are all dollies/bases stacked correctly, to an ideal height?

  • Is all fire walk-way routes within the department clear from any obstuctions?

  • Are all fire Extinguishers in good condition within the department, and clear from any obstructions?

  • Are all fire call points in good condition, not blocked or obstructed?

  • Is there any other concerns found at the time of safety audit in the home shopping department?

Warehouse/Delivery Yard

  • Are all warehouse fridges in good condition, visible fire alarm sirens/sprinkler systems in place?

  • Are all warehouse freezers in good working condition, with no buildup of ice, or Icey surfaces, causing a slip hazard?

  • Does all freezers used by colleagues throughout the warehouse, unlock from the inside, or does the audible/siren warning alert work?

  • Are all fire extinguishers in good condition, in the correct position, with the correct identification plate?

  • Are any fire extinguishers/fire call points obstructed or blocked (put in identification Number if blocked/obstructed)

  • Are all fire doors held open with mag locks, and do the doors close freely?

  • Do all fire doors have correct signage, ‘fire door, keep clear’

  • Is all first aid kits, clearly visible, and stocked correctly?

  • Are all fire route doors clear?

  • Are all fire routes clear?

  • If applicable are delivery bay shutter doors left open, unattended, potentially causing a hazard for colleagues/visitors to fall off the delivery dock?

  • Is PPE being worn around the warehouse by colleagues, management & visitors?

  • If applicable are all compactors in good working order, with no visible damage or known damage, which could potentially harm the operator?

  • If compactors no longer in use, has it been isolated/removed?

  • Is there any eye wash cleaning stations next to the charging points for the PPT/Reach forklifts?

  • Are the delivery gates locked/closed at the time of inspection, if not, explain why?

  • Is the delivery dock in good condition, with no visible damage?

  • Is the delivery lift in good condition, with no visible damage?

  • Is there any damage to the PPPT, Forklifts & Reach fork lifts, if so, list below.

  • If deliveries are under a canopy, does it have height identification plates?

  • Are all hired fridge/freezer containers in good working order, with working lights inside?

  • If applicable are all gas bottles safely secured in a locked cage, and is there a build up of gas bottles?

  • Are fireworks stored in the correct container, with correct identification signage visible?

  • Any structural damage to perimeter fence/walls/gates?

  • Are all pallets stacked correctly, away from the building, and not too high, these should be no more than 20 high.

  • Are all fire walk-way routes clear within the yard area & accessible in the event of a fire?

  • Are there any trip hazards within the warehouse/yard?

  • Is there a fire evacuation chair on the mezzanine floor?

  • Does all the fire walk-way routes have clear painted markings, with correct signage/directions?

  • Are Hi-Viz signage for entering areas where PPE is required, visible?

  • Are Hi-Viz vests made available for colleagues, management & visitors whilst entering a Hi-Viz area?

  • is there any known health & Safety issues raised by colleagues in the warehouse department?

  • Are battery charging areas clear of combustible materials?

  • Are all pallets in the racking free from damage, with stock wrapped securely to the pallet?

  • Is there any additional concerns found within the Warehouse department at the time of inspection?

  • Are all near-misses/accidents being reported within the department?

  • Is the COSH cupboard locked in the Cleaning room?

  • Is there any concerns in the Cleaning room?

  • Is there any other concerns found at the time of safety audit in the warehouse/yard?

Shop floor/Café/Customer Toilets

  • Are all fire extinguishers around the shop floor up to date, not blocked or obstructed?

  • Are the zero tolerance posters being displayed in customer area's?

  • Are all fire call points in good condition, not blocked or obstructed?

  • Is top stocking to company policy?

  • Are all fire doors clear from all obstructions, and accessible in case of a fire?

  • Is all first aid kits, clearly visible, and stocked correctly?

  • Are all fridges/freezers on the shop floor clear from any damages/leaks, which are potentially causing a hazard?

  • Are all kickplates on fridges/freezers in good condition?

  • Are all shelving throughout the store, in good condition, with trolley bump rails in place, Rodent guards in place, and crash bar end bungs installed?

  • Is the shop floor free from any slip/trip hazards?

  • Are all clothes racking within George, in good condition, with no damage, potentially causing hazards?

  • If applicable are all changing rooms in good condition, with no damage to doors, mirrors, seats?

  • If applicable do all changing rooms doors securely lock?

  • Are all seating area’s adequate around the store, safe, sturdy?

  • Is the temperature within the store reasonable?

  • Has all pallets been removed from the shop floor at the time of inspection?

  • Is the cleanliness of the store reasonable?

  • Are all service tills, and self-scan tills in good condition, with no defects, or visible damage?

  • Are all bakery machinery/equipment in good condition, with no known defects?

  • Are all rotisserie machinery/equipment in good condition, with no known defects?

  • Do all fire doors throughout the shop floor, open & close freely if you're able to check?

  • Are all customer toilets in good condition, with no known, or visible damages?

  • Does the toilet facilities have working hot/cold water, and does all the hand dryer’s work?

  • Are anti-fatigue mats in place, behind the customer service desk & Kiosk, if they are, record the condition?

  • Are all slip mats available & in good condition around the flower purchase area?

  • Are all thumb lock mechanisms installed correctly on the doors, and twist to the diagram above?

  • Are all restricted areas/doors around the shop floor kept closed, secured?

  • Are all plug sockets around the store fitted with child safety plugs?

  • Are all equipment within the café area, in good condition, including tables/chairs and other machinery for public/colleague use?

  • Is all ASDA owned appliances in the CAFE/Restaurant in good condition?

  • If applicable are all child rides in good condition, safe to use/operate?

  • Are all disability scooters in good condition, adequate amount of availability, safe to use/operate?

  • Does the cleaning stations have a good amount of D10, blue roll & hand gel?

  • Are all ceiling tiles/vent systems in good condition, with no visible damage, or raised tiles?

  • Does all ceiling lights work, with adequate amount of lighting?

  • Are all PAT tested equipment up to date?

  • Are all stairways, elevators, escalators in good condition, with no noticeable or reported defects, and displayed with correct signage?

  • Any known health & safety issues raised by members/non-members whilst doing your walkaround?

  • Is there any other concerns found at the time of safety audit around the shop floor?

Colleague canteen, locker rooms, toilets, offices

  • If applicable are all post office equipment in good condition?

  • Are the doors for the cash office secure?

  • If applicable is the extractor fan area, for the old colleague canteen clear, with no obstructions?

  • Are all fridges in good condition and clean in the colleague canteen?

  • Does the canteen have anything under the sinks which should not be there?

  • Are all colleague tables and chairs in good condition, with no visible damage/defects?

  • Does all electrical items in the canteen work, and have a PAT Tested sicker?

  • Does the sink have hot/cold water in the canteen?

  • Are fire evacuation chairs available on the top of all stairwells?

  • Are the floors clear of all slip/trip hazards?

  • Are the locker rooms in good condition, with all lockers clear from any damage?

  • Are the colleague toilets in good condition, clean, & with working Hot/Cold water?

  • Does the store have an up to date GMB board, with relevant information?

  • Is the well-being room in good condition, and is it made available to all colleagues?

  • Is the training room in good condition, all tables/chairs and equipment in good working order?

  • Are all first aid kits correctly stocked, and visible?

  • Is the meeting room in good condition, tables/chairs and equipment in good condition?

  • Are all fire extinguishers visible, in the correct place, with up-to-date service?

  • Is all equipment within the offices in good condition, including: tables/chairs?

  • Are lifts for colleagues in working order, with ‘do not use in case of fire signage?

  • Are all FM Compliance forms up to date?

  • Any known issues form colleagues under health & safety?

  • Are all cupboards kept closed?

  • Is there any other concerns found at the time of safety audit around the offices or colleague area's?
