Step -1-

  • Carefully read all notes on the Doka Drawings.

  • Once the concrete has reached 3,000 psf. you may prepare to jump the platform.

  • All fall protection has been inspected.

  • Inspect the platform for damaged wood or unsafe conditions.

  • Remove any loose item or secure them firmly.

Step -2-

  • Remove ties.

  • Remove all nails from openings.

  • Cut all bolts on embeds.

  • Prep upper safety hand rails.

  • Strike the formwork (strip the formwork)

  • Make sure all stripping corners are fully engaged.

  • Make sure formwork is secured.

  • Make sure the correct signage is in place ( red tape, fall protection sign, scaffold tags, ext).

Step -3-

  • Inspect all lifting points and rigging before you use it.

  • Attach the lifting chains or rigging

  • No ones is permitted to "ride" on the platform.

Step -4-

  • Reposition the unit (platform and formwork).

  • Once the platform is repositioned, before the rigging is cut loose, make sure the pawls are positioned correctly into position.

  • Release rigging.

  • Correct safety hand rail and safety net.

  • Baker Concrete, Competent Person signature.
