Guidelines

  • Spend Approximately 15 minutes touring area to view items listed below. Total Observation process should take about 20 minutes from time of pick-up to turn. Mark appropriate column s and add number of boxes in total section of each category. mark N/A column if you don't observe the behavior condition.
