Exposure control plan

  • Does the facility have an established written Exposure Control Plan, accessible to team members, to eliminate and minimize team member exposure to Bloodborne pathogens? If yes, is this plan available for review?

  • Is the exposure control plan reviewed and updated at least annually or when necessary due to changes in tasks or procedures that affect occupational exposures? If yes, is there a process to identify the individual responsible for updating the plan?

Exposure Determination

  • Has an exposure determination been prepared for team members with potential occupational exposure?

General Requirements

  • Are universal practices utilized to prevent contact with blood and other potentially infectious materials?

  • Are engineering and work practices controls used to eliminate or minimize team member exposure? If engineering and work practice contrails are not sufficient, is personal protective equipment utilized?

  • Are engineering controls examined and maintained on a specified schedule to ensure effectiveness? If yes, who is responsible for the schedule and inspections?

  • Are team members provided with hand washing facilities that are readily accessible to all team members?

  • When hand washing facilities are not feasible, are team members provided with antiseptic hand cleanser and clean cloth/paper towels or antiseptic towelettes?

  • When team members are required to come in contact with Bloodborne pathogens or other infectious materials, are they immediately required to wash their hands after removal of gloves or other protective equipment?

  • Are all contaminated needles and contaminated sharps properly disposed by following established procedures?

  • Is eating, drinking, smoking, handling contact lenses and applying cosmetics or lip balm prohibited in work areas with the likelihood of occupational exposure to bloodborne pathogens or other infectious materials?

  • Are all equipment, working services and work environments cleaned and decontaminated after contact with blood or other infectious materials?

Training Requirements- to handle infectious materials

  • Are team members trained and provided information on bloodborne pathogens and other infectious materials?

  • Is training provided at the following times:<br> • At the time of initial assignment to tasks with potential of occupational exposure<br> • At least annually after the initial training<br> • Process changes or modification of tasks

  • Does the training program contain the following elements:<br> • An accessible copy of 1910.1030<br> • General explanation of epidemiology and symptoms of bloodborne diseases<br> • Explanation of modes of transmission of bloodborne pathogens<br> • Explanation of written Exposure Control Plan and method to obtain a copy<br> • Explanation of appropriate methods for recognizing tasks and activities that may<br> involve exposure to blood and other infectious materials<br> • Explanation of use and limitations of methods to prevent or reduce exposure<br> • Information on types, proper use, location, removal, handling, decontamination <br> and disposal of personal protective equipment<br> • Explanation of proper selection of personal protective equipment<br> • Information on hepatitis B vaccine<br> • Procedures to contact emergency personnel<br> • Procedures to follow if an exposure occurs<br> • Post exposure evaluation and follow-up<br> • Explanation of signs and labels and color coding<br> • Question and Answer session with person conducting training

Recordkeeping

  • Are all records maintained and accurate for team members with occupational exposures?

  • The records shall include:<br> • Name and team member identification number<br> • Copy of team member's hepatitis B vaccination statues with dates of all <br> hepatitis B vaccinations and medical records relative to the team member's <br> ability to receive vaccinations<br> • Copy of all examinations, medical testing and follow-up procedures results<br> • Copy of any healthcare professional's written opinion<br> • Copy of any information provided to the health care professional

  • Are all medical records kept confidential and not disclosed or reported without team member written consent?

  • Are records maintained for at least the duration of employment plus 30 years?

  • Are training records maintained and contain the following information:<br> • Dates of the training sessions<br> • Contents or summary of training sessions<br> • Names and qualifications of persons conducting training<br> • Names and job titles of persons attending the training sessions

  • Are the training records maintained for at least 3 years from the date on which the training occurred?

  • Are all required records available for review upon request?
