Fall Protection

  • Are floor holes covered and clearly marked?

  • Working at heights?

  • Is fall protection being used at heights of 6' and greater?

  • Has the fall protection been inspected?

  • Harness properly worn?

  • Proper anchor point used?

Ladders

  • Are ladders being used?

  • Have the ladders been inspected and maintained?

  • Are the ladders being used properly?

  • Is the extension ladder secure 3' above the working surface?

  • Are tools being stored on top of the ladder?

  • Is the set up of the ladder on a stable surface and clear of debris underneath?

Lock Out Tag Out

  • Lock out Tag out needed?

  • Has the employee been trained to perform LOTO?

  • Has the LOTO Tag and or log been filled out and correspond with the lock that is installed?

  • Have the locks and locking devices been properly installed?

  • If the lead Has LOTO responsibility, has this been communicated to all workers on site?

  • Is the operator using a safe travel path and obeying speed limits?

Excavations and Trenching

  • Any Excavation work being performed?

  • Locates called in?

  • Is the trench or excavation red barricaded off and maintained?

  • Is there surface traffic exposure?

  • Less than 5' deep or shoring being used

  • Spoils 2' from edge of trench?

  • Is there egress within 25' of employees?

  • Any unsafe surface conditions? (water, fissures, ect)

Confined Space

  • Working in confined spaces?

  • All personnel are trained?

  • Permit filled out properly?

  • Air samples taken & recorded?

  • Rescue plan in place if needed?

  • Have the hazards been abated?

Powder / Gas Actuated Tools

  • Are Powder actuated tools in use?
