Personal Protective Equipment (PPE)

  • Not wearing rubber gloves and sleeves is a Bowlin CESR violation. Employee's GF and Jon Keith are to be notified immediately. A disciplinary action form must be completed .

General Electrical Work Requirements

  • Dielectric test dates are current for the bucekt trucks assigned to the crew?

  • Call Eddie Jacobs immediately and schedule a dielectric test. If the truck has not been tested within 96 hours (4 days) of the violation, the truck will be shut down until a test can be completed.

  • Outrigger pads being used when outriggers are deployed?

  • Address the issue with the foreman. If the truck does not have outrigger pads, contact Brandon Adkins in the Warehouse, give him the truck number(s) and ensure he orders pads for the truck.

  • Employees are staying off of the bucket truck while the boom is in the "Hot work Zone"?

  • Call and all-stop and address the severity of this issue. The employee and foreman will receive at a minimum a verbal reprimand which should be documented using the iAuditor disciplinary action template.

  • Employee Secured in basket? (PFAS being worn at 6' or higher)

  • (BUCKET TRUCK) A hand line has been attached properly to the pole?

  • Call an all-stop and address the importance of having a hand/rescue line attached to the pole. The work is not to proceed until a the handline has been installed

  • (BUCKET TRUCK) Materials are being raised/lowered with hand lines?

  • (CLIMBER) Has employee safely maintained three points of contact and maneuvered with Buckhoist/Supersqueeze/Rope Safety attached at all times?

  • Complete a Lineman Skills evaluation for the employee climbing

  • If using a chainsaw, is the employee using his Buckhoist/Supersqueeze and his secondary rope safety (double tie off Bowlin requirement).

Workzone Protection

Vehicles and Motorized Equipment

  • DOT Daily Log Books filled out for the truck(s) for that day?

  • Ensure employee knows Eddie Jacobs receives copies of the vehicle logs at the end of the week (Scanned, texted or given physical copy)

  • Ensure employee completes log book inspection prior to leaving the job site

  • Picture of the completed log book entry

  • (BUCKET TRUCK) Pneumatic and Hydraulic hoses have been thoroughly inspected?

  • (DIGGER) Pneumatic and Hydraulic hoses, winch line has been thoroughly inspected of the digger?

  • Loads are properly secured? (Look for loose/unsecured materials in the beds of trucks)

  • Vehicle housekeeping is acceptable. (This includes the cab and bed of all work vehicles).

  • What truck number is requiring immediate housekeeping?

  • Add a photo of the housekeeping issue

  • Trucks have a Spill Kit, First Aid Kit and Fire Extinguisher?

  • Have employee/foreman contact Brandon Atkins for immediate replacements.

  • Warning/Hazard Lights Operable?

  • Vehicle chocks in use while parked? (Chocks placed properly on both wheels for uneven grade)

  • Have you observed the vehicles in motion while completing your audit?

  • Seat belts are being worn by all occupants while in motion? (Not wearing a seat belt is a Bowlin significant violation)

  • 1st offense- documented verbal warning using the iAuditor disciplinary form. 2nd offense- written and signed warning. 3rd offense- minimum of 3 days off using Bowlin disciplinary form. Send signed copy of the form to safety and Rick Ryan in HR

  • Spotter was utilized when the vehicle was in reverse? (Spotter required when two or more employees are available)

  • 360 degree walk around completed prior to moving?

  • Hands free communication is being used when vehicle mobile?

  • If in motion, are all work vehicles leaving enough space between vehicles while in transit?
