Cranes and forklifts

  • Are all power lines marked with proper signage?

  • Is the crane operator certified?

  • Are crane check sheets being filled out daily before the start of the shift?

Ladders and Stairways

  • Have all ladders been inspected for defects?

  • Do all extension ladders extend 3' above the working landing?

  • Are extension ladders pitched at 1' out from vertical for every 4' height?

  • Are extension ladders secured in place at bottom and top of ladder?

  • Are all stairways equipped with stair rails and handrails?

  • Are stairways free of all debris, trip and fall hazards?

  • Are all step ladders being used properly?

Personal Protective Equipment

  • Are all employees using the proper eye protection (Z87 & SIde Shields)?

  • Are steel toed boots/shoes being worn by workers if they are required for this jobsite?

  • Are respirators being used on this site, and if so, have all employees been properly trained for proper usage, proper fit/medical clearance?

  • Are hard hats being warn?

Scaffolds

  • Is a competent person available and assigned to supervise the erection or disassembly of scaffolding?

  • Have all working surfaces 10' or higher been equipped with guardrails and toe boards?

  • Are all working surfaces complete with no holes, and clear of debris, slip/trip/fall hazards?

  • Are all toe boards, rails, gates, pit-tails, pads, jack's and/or wheels in place, inspected, and proper?

  • Are bakers scaffold assembled correctly?

Electrical

  • Have all outlets been checked for proper wiring and GFCI checked for proper operation?

  • Are all tools and equipment inspected for defects in cords and plugs on a pre-use basis?

  • Have all extension cords been inspected for damage to the plug, missing pins and/or cuts in insulated jacketing or strain relief?

  • Have all sources of electricity such as panel boxes, overhead lines, and temporary power been identified and properly marked?

  • Are gfi's being used?

General Fall Protection

  • Are control access ropes in place for leading edge work?

  • Are all floor openings larger than 2" in diameter, covered or guarded?

  • Are all workers exposed to falls 6' or more protected?

  • Are all employees trained in the proper utilization of Personal Fall Arrest Systems and its required components?

  • Are employees wearing harnesses correctly, and are the harnesses the correct size for the employee?

  • Are employees properly utilizing fall protection in all Mobile Elevated Work Platforms as required by this jobsite?

  • Are all rails in place?

  • Are all elevator and mechanical shafts covered?

  • Are CAZ signs in place?

Fire Prevention

  • Are all Flammable/Combustible liquids stored away from ignition sources?

  • Are all portable containers used for flammable liquids approved and provided with self-closing lids and flashback screens?

  • Is there an adequate number of properly charged fire extinguishers available?

  • Are fire extinguishers properly located within close 25' of all gas compressors and generators?

Hazard Communication

  • Written Hazard Communication Program is in place?

  • Written program includes a complete list of all chemicals being used on jobsite?

  • Are all chemical containers properly labeled?

  • Safety manual is readily available to all employees AND a current SDS is located in the log book for every chemical on list?

Tools and Equipment

  • Tools and equipment in good general condition?

  • Defective equipment tagged as such and removed from work area?

  • Tools and Equipment guards in place and in good condition?

  • Do air compressors have relief valves.

First AId/Emergency Services

  • First aid kits are readily accessible to all workers?

  • Properly trained first aid providers are onsite?

Housekeeping

  • Suitable containers with covers available for disposal of trash and debris?

  • Walkways, aisles, hallways and passage ways clear of trash, debris and materials?

  • Trash and debris containers are emptied at end of day, or as needed throughout the day?

  • Tools not in use are stored in job boxes?

  • Equipment not in use stored properly?
