  • *** The approach by the observer is to demonstrate genuine concern for the worker’s health and safety. Observers should focus on good practices as well as identifying unsatisfactory practices. Observations should be short and take no more than 15 mins. Review past observations for the area of work, and previous actions. Use consultation and discussion to identify action and gain commitment from workers to change work habits. ***

POSITION OF PEOPLE

  • o Are operators observing a safe exclusion zone around the load?<br>o Is the load being lifted over pedestrian walkways and can they be blocked during lifts?<br>o Operators aware of and away from crush hazards, either sideways or downward hazards?

PROCEDURES

  • o Are there procedures for standard repetitive lifts?<br>o Is the operator aware of the procedures and are they being followed? <br>o Is the crane operator holding a High Risk Licence or been deemed competent by a RTO?<br>o Is the person using slings and chains a qualified dogger or following a lifting procedure?

WORK PRACTICES

  • o Is the daily pre-start check book for the crane completed?<br>o Are barricades set up in the form of danger tape?<br>o Is a test lift completed to ensure the load is level, prior to moving the load a distance?

HOUSEKEEPING

  • o Is the path clear for the crane operator to walk?<br>o Is the lifting equipment hung up and not causing a trip hazard?<br>o If using a forklift or franner, can the load move sideways due to uneven surfaces?

TOOLS & EQUIPMENT

  • o Are all lifting equipment tag and tested on a regular basis?<br>o Do slings have any cuts or frays in them?<br>o Is the horn/bell and lights working on the crane?

PERSONAL PROTECTIVE EQUIPMENT

  • o Check Eyes & Face, Ears, Hands, arms, Feet & legs, trunk (is equipment being used appropriate, is it fit for purpose and in good condition)

MAIN AREA OF FOCUS: CRANES & LIFTING

    Note down any site, crane or lifting equipment hazards noted during the observations may include; ( parts of crane not working properly, is the compass on bridge cranes visible, is there appropriate lifting gear areas up out of the dirt and grease, any damage to cranes).
