JSA

  • Did employees meet you with the JSA before you entered the work area?

  • Are workers wearing snake guards? (If required by the customer)

  • Do employees have current and up-to-date H2S certification?

Pumps/Hoses

  • Are the pumps free of trash?

  • Is the containment valve closed on self-contained pumps? (and plugged, if required)

  • Is each pump equipped with a mounted, fully charged and inspected fire extinguisher?

  • Are all stabilizing jacks in use on the pump?

  • Are all stabilizing jacks pinned securely?

  • Are employees using team Lifting for objects over 50 lb?

  • Are the pump tires chocked?

  • Are the hoses free of damage or leakage?

  • Are the camlocks properly secured? (via pins, zip ties, or duct tape)

  • Are the couplings / connectors free of corrosion damage? (Imbrittlement)

Trailer/Deer Blind

  • Is the trailer / deer blind equipped with a fully charged and inspected fire extinguisher?

  • Is the trailer and deer blind free of trash?

  • Is the generator only being used for the AC unit, lights and company two-way radio?

  • Is the work area clear of trash and other unneeded parts/ fittings?

Company Trucks

  • Is the truck free from trash inside the cab and in the bed area?

  • Is the registration current on the truck?

  • Does the truck have any new damage?

  • Are trucks parked to avoid backing when possible?

  • Is the dashboard free of lit indicators that need to be addressed and fixed?
