Emergency Preparedness

Entrances and walkways

  • Is the flooring inside the offices sufficient? no trip, slip or fall hazards?

  • Are walkways clear of slip, trip or fall hazards?

  • Are there any uneven surfaces that could cause a fall?

  • Any matting or rugs that are used are they flat? (particularly around the edges)

  • What is the housekeeping like? In your opinion

Offices

  • Are there any particular trip hazards in the office?

  • Are the underneath of desks free from clutter? (to allow the user to move freely whilst at their desk)

  • Are all briefcases, bags and other types put away so as not to cause a trip hazard?

  • Are there any trailing cables?

  • What is the housekeeping like? In your opinion.

  • Have all users carried out a DSE Assessment?

  • Do all leads and cables look free from damage?

  • Are there any extension leads used?

  • If extension leads are used, are they plugged into another extension lead? (this is known as daisy chaining and is very dangerous because they heat up and could cause a fire)

Shop floor

  • Are there any identified spills?

  • Is there any equipment to clean spills? If so what is there to aid this?

  • What is the housekeeping like? in your opinion.

  • Observe a manual handling lift? have they lifted in the correct way?

  • Is a crane being operated?

  • Is the crane being operated in a safe and orderly manner?

  • Machine guarding is in place for machinery?

  • Are there any guards that are not in place?

  • Is there any maintenance being carried out on machinery?

  • Is the machine locked out?

  • Striking tools - are there any risk assessments for the use of hammers? is the right equipment available?

  • Hand tools - Are they in good condition? Are they fit for the task? Are they worn and in need of replacing?

  • Are there shadow boards in place?

  • Are there any spaces on the shadow boards?

  • Are there any sharp edges in the area?

  • Is the PPE suitable for the tasks? List what PPE you have found during this audit.

  • Ladders - do they have inspection dates? (usually found attached to the ladders)

  • Take a picture of one of the dates on the ladders?

  • Is the ladder in good condition?

  • Are there any ladders in the area that should not be there?

  • Is any electrical equipment in the area damaged?

  • Are there any extension leads being used in the area? do they have RCD's on (Residual Current Device)

  • What areas have you visited during this audit?

Chemicals

  • Are there any chemicals in the area that should not be there?

  • Are there any flammable chemicals in the area?

  • Is the storage of chemicals suitable?

  • Is there a COSHH Cupboard for chemicals?

  • Identify a chemical and then find the COSHH Assessment? is this clear and concise for the task?

  • Is there a list of chemicals that are used in this area?

  • Ask a person in that area, what they would do of there was a spill of XXX Chemical?

  • Are all chemicals labelled and clearly identified?

Maintenance Activities

  • Control of contractors is in place?

  • Permit to work is in place for non standard activities?

  • Identify a permit that has been issued and follow through the process to closure.

  • Working at height, Is there anyone at this time working at height?

  • Is there any scaffolding onsite? if so does it display a current scafftag inspection?

  • Are all fixed based ladders locked off when not in use?

  • Are MEWPS in use? (Mobile Elevated Work Platforms)

  • Lock out Tag Out (LOTO) Is any machinery locked off?

  • What machine is locked off and is it clear who locked the machine off?

  • Follow the lock out process back to the LOTO board, is all the information documented, as to who has locked what off?

Pallet Racking

  • Does the pallet racking have a notice stating how much weight each bay can take?

  • Do any of the supporting beams or columns look bent or unusual?

  • Is there any visual damage anywhere else or looks unusual?

  • Is there crash protection installed for all of the uprights?

  • Are weight loads clearly marked?

  • Are all the loads secure? Does anything look wrong or unusual?

Slips, Trips and Falls.

  • Is there evidence of trailing cables across walkways?

  • Are all walkways clearly identified?

  • What is the state of housekeeping in the area? In your opinion.

Powered Mobile Work Equipment.

  • All users of Forklifts are trained? Identify an driver and follow up on their training record.

  • Have the pre-use inspection been carried out for that shift?

  • Find a forklift without an operator, is the key in the ignition?

  • Is there sufficient signage in place where forklifts are used?

  • Are Forklift drivers using their horns when going around blind corners or doors?

  • Are the `forklift trucks damaged? Take photos of any damage.

  • Are forklift drivers aware of the 3 meter rule?

  • Observe personnel , are they all wearing highly visual workwear?

  • Observe Forklift drivers are they driving safely and aware of pedestrians?
