General/Environmental Safety

  • Are safety signs/warnings posted where needed?<br>

Housekeeping

  • Are aisles and passageways kept clear and clean?<br>

  • Is the trash/clutter all taken care of (including excess of combustibles)?<br>

Walking/Working Surfaces/Stairs

  • Are surfaces free of slip/trip/fall hazards (spills, ice, uneven ground, etc)?

  • Are stairs with 4 or more risers equipped with a stair rail (top rail 30"-34")?<br>

  • Do handrails have at least 3 inches of clearance between the handrail and the wall surface they are mounted on?<br>

  • Do stairs have the proper "Rise of 7" and a run of 11"?

Electrical

  • Are all electrical panels closed and also locked if in general access area(Hallways, classrooms etc.)?<br>

  • Are all extension and electrical cords in good condition (no cuts, tape, no missing ground prong)?<br>

  • Are extension cords properly used and not as a permanent wiring solution?<br>

  • Are GFCI's utilized when working outdoors or in damp locations?<br>

  • Is access to electrical panels clear and not obstructed (36" clearance in front of panels)?<br>

  • If space heater is in use, does it abide by all safety rules ( 36in from combustible, UL approved, etc)?

Ladders

  • Are the top two steps of a ladder not being utilized by employee and on stable ground?<br>

  • Are ladders inspected and maintained in good condition?<br>

  • Are extension ladders extend a minimum of 3 feet above top of upper landing surface when in use?

  • Does employee maintain a three (3)-point grip on the ladder at all times and carry tools/equipment on a belt or hoist up?<br>

  • Is extension ladder secured at the top or held by another person at the base when in use? <br>

Personal Protective Equipment(PPE)

  • Are protective goggles or face shields provided and worn where there is a danger of flying particles or corrosive materials?<br><br>

  • Are approved safety glasses worn at all times in areas where there is a risk of eye injuries?<br>

  • Is hearing protection utilized when noise levels exceed 85 decibels(TWA)?<br>

  • Are protective gloves, aprons, shields, or other means provided where employees could be cut or where there is reasonably exposure to; hazardous chemicals, blood, or other potentially infectious materials?

Fall Protection

  • Is fall protection provided to employees working above 4' from the lower level?<br>

  • Is all fall protection equipment inspected and in good condition?<br>

  • Do open-sided surfaces, 4' or higher, have standard railings consisting of a top rail (42" +/-3") and midrail?<br>

  • Are floor openings guarded by a cover?<br>

  • Is employee on aerial lift wearing harness/lanyard connected properly?<br>

Tools and Equipment

  • Are all power tools provided with appropriate safety guard?<br><br>

  • Are abrasive wheel work rests installed and kept adjusted to within 1/8th inch ?<br>

  • Are abrasive wheels tongue guards adjusted to within 1/4" inch from the wheel?

  • Are all tools and equipment periodically inspected and in good working condition (no frayed cords, rust, missing parts etc.?<br>

  • Are all forklifts, scissor lifts, aerial lifts ect. have daily and yearly inspections completed?

  • Are all shelves and stacked material secured to prevent from falling over?<br>

  • Are all employees operating forklifts, scissor lifts, aerial lifts properly trained?

Hot Work/Compressed Gas Cylinders

  • Are all hot work, outside of approved areas, utilizing a hot work permit?<br>

  • Are cylinders legibly marked to clearly identify the gas contained?<br>

  • Are cylinders stored or transported in a manner to prevent them from creating a hazard by slipping, falling, or rolling?<br>

  • Are valve protectors/caps always placed on cylinders when cylinders are not in use?<br>

  • Are cylinders stored at least 20 feet away from combustibles?<br>

  • Are fuel gas and oxygen cylinders stored a minimum of 20 feet apart or separated by a 1-hour firewall?<br>

Hazardous Chemicals and Lab Safety

  • Are SDS's easily accessible where hazardous chemicals are being utilized?<br>

  • Are all containers for hazardous substances labeled with product identity and hazard warnings?<br>

  • Are propane tanks stored outside (maximum allowed per building is 2lbs)?

  • Are flammable and toxic chemicals kept in closed containers when not in use?<br>

  • Are emergency eyewash/shower installed and regularly inspected and accessible?<br>

  • Are corrosive stored properly, below eye level?<br>

  • Is each fume hood in good condition and have proper flow rate?<br>

  • Are refrigeration units containing flammables approved for flammable storage?<br>
