Nature of work

  • Have site-specific hazards introduced by these operations been identified and mitigated?

  • Has a lift plan been communicated to all workers involved in the operation, and appropriate hose-hands briefed?

  • Has pedestrian and traffic flow around operations been controlled?

  • Has an acceptable area been set up for safe receiving of concrete delivery vehicles?

Set-up stability

  • Is the boom arc free from obstacles including overhead hazards?

  • Are outriggers (if needed) used to level the machine correctly, fully extended and used on firm footings?

  • Is the machine operated on level ground?

Operator competency

  • Is there evidence of company or manufacturer’s training?

  • Is the operator familiar with machine?

  • Are hose-hand operators knowledgeable about safe practices and have a predetermined method of communication with the operator?

Onboard documentation

  • Are there operator/manufacturer’s instructions?

  • Are logbooks available?

  • Are pre-start checks in line with the above?

  • Are operator instruction labels (decals) legible on all controls?

  • Are there records of inspection?

Pipeline set-up

  • Unnecessary bends are avoided

  • Quick release pipe clamps, and connections have locking pins and safety chain/wire in case of blowout.

  • Clamps are marked with operating pressure, and are compatible with the pipeline set up.

  • The correct concrete pump mix is being used.

Safe operation

  • Is the decontamination of soil undertaken by a person with CoC?

  • Is the operator able to safely control the pump and boom, while maintaining contact with hose-hands?

  • Are there emergency procedures that have been communicated to the workers before pumping begins?

  • Can the operator identify demonstrate the emergency stop/ cut-out devices?

  • Are hopper guards, that prevent access to moving parts, installed?

  • Are they connected with an interlock cut-out switch that stops the moving parts when lifted (usually for cleaning)?

  • Are there any evidence of excessive leaks?

  • Are hoses and protective coverings intact?

Machine warning devices

  • Are movement-limiting devices/limit switches present and active? (Ask for a demonstration.)
