Was atmospheric hazard testing done in the proper order? (oxygen, then combustible gases and vapors and then toxic gases and vapors)?

Have the attendants been trained in the hazards of the entry, to recognize behavioral effects of hazard exposure in authorized entrants and to call for rescue services if necessary?

Has the employer evaluated the rescuers or emergency service’s ability to respond in a timely manner?

Are fall protection gears (harness and lifelines) used?

Is self-contained breathing apparatus available if needed?

Are the tools to be used explosion proof?

Has the entry supervisor signed off on the permit indicating that entry conditions are acceptable?

Are completed entry permits posted near the confined space entrance for all involved to observe?

Confined Space Pre-entry Inspection Checklist

Use this checklist before entering a confined space. Start by filling out general information about the confined space, its type, how often it is used, and the work to be performed inside. List down names of the people involved in the work and classify if it is permit-required or not. Next check if the area is free from hazards and ensure that the atmosphere is tested carefully. Evaluate if the workers are competent and trained to perform the job and if proper protective equipment (safety harness and lifelines) are worn properly. Lastly, ensure that permits are posted in the entrance of the confined space and show the names of the persons qualified to perform the job. Complete the audit by citing any safety recommendations.
