Welfare & Canteen

  • The correct number of toilets are provided and maintained/cleaned

  • Changing and drying facilities are provided and maintained

Site Questions

  • If lifting operations ongoing, observe to ensure that it is done safely.

  • Stand and observe plant/tipper/FLT movement to see if the operator looks behind when reversing.

  • Are banksman being used to control vehicle movements?

  • Vehicles have reversing warning alarms where appropriate

  • Are all safety guards and devices fitted and working correctly

  • Is the appropriate PPE being worn by those on site in accordance with the Risk Assessment for the activity and Site Rules

  • Is there a site securely fenced to prevent unauthorised access

  • Is access and egress to site controlled

  • Appropriate site safety signage is in place

  • Assembly points are clearly marked

  • Is the site tidy and are materials stored safely

GS6 Procedures

  • Are there OHPL on site

  • Have goalposts and bunting been set up specific to the requirements of the DNO

  • Have barriers been set up 5 metres either side of the raod/track to prevent unauthorised access

  • Is there suitable signage displayed to warn of the danger

  • Are traffic movement activities next to/under the OHPL controlled by a banksman

Excavations

  • Are there excavations on site?

  • Properly designed and maintained support systems are in place. >1.5m deep

  • Safe secure access to the excavation is in place >1.5m deep

  • Sufficient barriers are in place to stop persons accidentally falling into the excavation. All.

  • Plant, materials and spoil are stored away from the excavation to minimise the possibility of collapse. All.

  • The > 1.5m deep excavation is inspected and the results recorded by a competent person on a regular basis.

Use of hand tools

  • Are hand tools being used on site?

  • Is anyone using a tool that can create a Hand, Arm, vibration issue?

  • What is the tool? (Type answer)

  • Ask the worker how long s/he can spend on the tool. What was the answer?

  • Does the tool require ear protection? (Non-scoring question)

  • Observe the tool being used. Did the operator use hearing protection?

  • Are cables and leads protected from damage

  • Are tools and equipment checked by users and in a serviceable condition

  • If there is a generator on site, has it been grounded?

Scaffolds

  • Is there a scaffold on site?

  • Inspect the scaffold from top to bottom to top. Is it safe to use?

Step/Ladders

  • Are there Step/Ladders on site

  • Are in good condition

  • Are all step ladders Class 3?

  • Do ladders rest against a solid surface and not on fragile or insecure materials?

  • Are secured to prevent slipping sideways or outwards

  • Ladders extend the correct distance above the working platform? 1 in 4 ratio.

Working at Height

  • Is there work at height on site?

  • Is there a method for lifting heavy equipment up to the working platform? (Where required)

  • Are harnesses available and used

  • Is there adequate edge protection

  • Is work allowed under areas with persons working at height?

  • If the MEWP is in operation, question the operator of the emergency lowering procedure.

  • Is the SWL clearly marked

  • Are operatives trained and competent (PASMA, IPAF, IRATA)

  • Check the operator's MEWP IPAF certificate.

  • Is the MEWP serviceable and in good condition? (Safety bar. Oil levels, Emergency lower lever, tyres, etc)

Environmental
