Instructions: This form is used to track the "open air" plastic crates to be sanitized for the following day. The personnel pulling crates is to leave them on a skid in the bones room for the sanitation crew to clean that night. The personnel pulling the crates shall record the date, number of crates pulled, number of crates found broken, and initial. Crates which are broken are those that have two or more plastic sides broken off, or which have a single jagged broken piece which which could present a foreign material hazard in the facility. They shall be discarded and recorded. The sanitation crew shall ensure all labels are removed during the cleaning process using a pressure washer. The sanitation crew shall discard and record any additional crates identified broken, and initial they have cleaned the crates
