  • Greet the crew in a friendly manner and explain the reason for the visit.

  • Stop the job and require everyone to conduct a safety pause as a group before continuing work.

Install

  • Are Installers working at this job?

  • Is anyone on the roof without being tied in properly?

  • Who is not wearing/using fall protection?

  • Is everyone staying within 30 degrees from front of the anchor point?

  • Is anyone crossing lifelines?

  • Is anyone using a positioning lanyard by itself?

  • Could the employee walk off the roof in any direction, if they wanted to, without re-adjusting their rope grab?

  • Are tools secured, with no risk of falling or being dropped to ground level?

  • Any visible defective fall protection equipment on site?

  • Take a photo of all of the roof access ladders that are in use.

  • Are the access ladders secured properly?

  • Are the access ladders 3' above roof eave?

  • Are employees using the ladder maintaining 3-points of contact?

  • Are they using fall protection?

Electrical

  • Are there electricians working here?

  • Are the electrician's using a ladder other than the access ladder to the roof?

  • Are the electricians maintaining 3 points of contact when using the ladder?

  • Does the ladder have bullhorns? (If able)

  • Does the ladder have blue rubber covers on the side rails? (If able)

  • Is anyone working on energized equipment without arc flash gear on?

  • Was the meter pulled using arc flash protection?

  • Are the electricians in the attic?

  • Is the attic considered a confined space?

  • Was a pre-assessment completed and available for viewing?

  • Are pit boards in use?

General Safety

  • The work is site tidy, continuously maintained, and free from preventable trip hazards?

  • Are employees wearing hardhats where required? (anywhere an overhead hazard exists)

  • Cut-resistant gloves when handling sharp objects?

  • Are employees wearing safety glasses?

Vehicle Safety

  • Take a photo displaying vehicle setup.

  • Are the vehicles organized and well maintained?

  • Do the vehicles have 2 cones? (1 in front and 1 in rear of vehicle unless parked in road, cones should be on side nearest traffic)

  • Are the vehicles chocked? (pitched surfaces only)

  • Is the Driver's Vehicle Inspection Report (DVIR) book filled out for the current week?

  • Is the area around the vehicles neat and organized?

  • Are the fire extinguisher and first aid kit readily accessible?

Communication

  • The crews are communicating clearly and effectively.

  • The lead is leading and takes responsibility for his crew's safety.

  • I have reviewed these findings with the leads or any other member of the crew?

  • Thank the crew sincerely for their hard work before leaving.
