Overburden Excavator Checklist

WORK AREA

HSEC

  • Have there been any safety issues/Incidents on shift?

  • Are all the COVID controls being adhered to (cribhuts/LV/mobile equipment?)

  • Communication of the OCE Report and unknown hazards/conditions

  • Have clear instructions been communicated to CMWs on relevant tasks

  • Are there any JSAs in place, if yes is it signed off by all operators and OCE if applicable

  • All light vehicles pre-starts complete and submitted

Exposed Edge Protection Confirmation

  • Has any safety berm, high risk berm or catch berm been removed during the shift? If yes, was authorisation sought from area supervisor?

Haul Roads and Ramps

  • Traffic signage in place and maintained?

  • Are all Haul roads complaint (eg High Risk bunds, width 35m, Grade 10%, adequately maintained )

  • Does the circuit have proper drainage and correct cambers?

  • Road surface condition- does it need to be scarified to remove the shine/ smooth

  • Do haul roads have maximum standard 4% cross. Grade?

  • Are all cross grades complaint and not identified as “non complaint “ in the monthly cross grade report in the circuit?

  • For in-pit roads on coal benches with a cross fall between 4-8%, what controls are in place to adequately manage the hazard

  • Corner cambers in place and centre bunding on downhill high speed

  • Delineation in place? ( height between 1.5-2.4m, 100m spacing on straights, 50m spacing at corners)

Bench and Loading area

  • Excavators- dig face less then cab height, dig bench less the 4m

  • If no, what are the controls?

  • Angle/grade of the dig bench, is it flat/level?

  • If no, what are the controls?

  • Lighting plants setup and have the appropriate bundling and delineation in place?

  • Designated LV park up area established and maintained?

  • AMAs in place with bundling and clear visibility?

  • Open Edges- Have you checked that the required controls are in place and are effective above and below dig units

  • Is pit edge and high wall standoff bundling adequate?

  • Is drill prep required ready for D&B?

  • High wall cracking or water issues evident?

  • Are the work area being excavated to design?

Dump Area

  • Authorised dump plan in place on design?

  • Is the dump square? eg 90degree corners?

  • Is the dump face straight? Not rounded?

  • Area dump heights no more than 20m and if so, is dumping short and dozing occurring?

  • What other controls are in place?

  • Are trucks and dozers complying with correct dumping requirements with maintaining separation? (Eg positive communications with dump dozer, not using tip head as a stop?

  • AMAs in place with bundling and clear visibility?

  • Designated LV park up area has been established and maintained?

  • Is water present at the toe of the dump and if so, is dump short and dozing occurring?

  • Lighting plants set up and have the appropriate bundling and delineation in place?

  • Access to the toe of all dumps are bundled off to all traffic?

  • Are the dump bund walls a minimum half wheel height to the largest vehicle on circuit?

  • Is a JSA in place?

  • Can the dump manage rejects? If no why?

  • Has a prestart been completed for lighting plants as per site CVM CHL mobile lighting plant / Generator Prestart inspection checklist?

New Work Area and Relocations

  • If relocating within the next 24hrs, has the route been inspected and discussed with operator?

  • If relocating in the next 24hrs, has the area been inspected and made compliant?

  • Have intersection checklists been completed?

Crib hut Area Housekeeping

  • Is there a high risk bund between truck park and crib hut ?

  • Has housekeeping of the crib huts been completed?

  • Is the crib hut stocked for the next shift? (Water, coffee, milk, earplugs etc)

  • Are all medium and LV park ups compliant?

  • Crib hut compliance checklist completed? ( First aid kit, Fire Extinguisher, Electrical test and tag)

  • Crib hut/ Go line adequate?

  • Has prestart been completed for the generator as per CVM CHL Mobile lighting plant / Generator Prestart inspection checklist CVM-CHL-0045

Water Truck Management

  • Water trucks communicating with haul truck operators when entering circuit?

  • Have the Water Truck Operators reduced watering (rate) approximately 30m before inspections, stop signs and access points.

  • Has the water truck operator communicated the “DRY LINE” and “WATERED LINE”?

  • NIGHT SHIFT OPERATIONS- has the water truck been parked up by 10pm?
