Are all personnel in a minimum ANSI level 2 rated vest?

Are spotters being utilized in tight/congested areas?<br>

Are back up alarms in place?

Are adolfo flags being used for trailer loading/off loading?

Are wheel chocks being used on the equipment and vehicles that require it?

Are employees walking/working safely around mobile equipment?<br>

Are attachments grounded and secure when the operator is out of the cab?<br>

Have DVIs been completed for any equipment that is involved in the operation?

Are equipment/vehicles parked at a safe distance from the operations?

Are employees protected from 3rd party exposures? (traveling public)
