Pre-Trip Inspection

  • Check off the boxes as you inspect that part. Should any defects be found IMMEDIATELY inform your supervisor who will instruct you on whether or not to fill out a legal DVIR.

  • Review previous post-trip driver vehicle inspection report - no issues or the issue has been resolved.

  • Service Brakes, including trailer brake connections

  • Fluid leaks in the engine compartment

  • Fluid leaks from the under the vehicle or attachments? e.g. digger derrick or bucket truck components.

  • Attachments or other apparatus coupling points are in safe working condition?

  • Parking (hand) brakes

  • Steering mechanism

  • Lighting devices and reflectors

  • Tires

  • Horn

  • Windshield wiper(s)

  • Rear-vison mirror(s)

  • Coupling devices

  • Wheels and rims

  • Fire extinguishers

  • Spare fuses

  • Warning devices

  • Cargo properly distributed and secured

  • Tailgate, tailboard, doors, tarps, spare tire, other equipment, and cargo securement system are secured

  • Cargo or any other object does not obscure the driver's view, interfere with the driver's movement, prevent easy access to emergency equipment, or prevent a quick exit from the cab

  • Remarks - Enter any defective equipment here. Report all defective equipment immediately to supervisor. Supervisor will instruct you to fill out a DVIR form if necessary.

  • Condition of vehicle is satisfactory

  • Is this a bucket or digger truck?

  • Has the pre-flight been performed?

  • Please perform the pre-flight. 1910.67(c)(2)(i)

  • This Section is complete. When this Vehicle is returned to the shop please complete the DOT required Post-Trip portion of this form.
