Personal Protective Equipment (PPE)

  • Are employees wearing the appropriate footwear? (Leather work boots over the ankle)

  • Is hearing protection available on site?

  • Is there are flash equipment on site if appropriate?

  • Are insulated gloves current on testing if appropriate?

  • Have employees been medically examined, trained, and fitted for all required respirator use?

  • Have employees signed an appendix D letter for voluntary use of respirators?

Powder Actuated Tools

  • Are powder actuated tools used?

  • Do employees have their card with them during use?

  • Are signs posted indicating powder actuated tools are being used?

  • Are employees using the tool properly with the appropriate PPE. (IE face shield. hearing protection, gloves)

  • Are employees announcing a warning before firing the tool?

Scaffolding

  • Is there a competent person available for inspections?

  • Has the safety department inspected the installation of the scaffolding?

  • Is fall protection used at 6 feet? (IE Guardrails or Personal Fall Arrest System)

  • Is the scaffolding setup on a firm foundation?

  • Is the scaffolding fully planked with no opening greater the 2 inches?

  • Are scissor lifts used and have employees been trained on there use? (Toolbox talk is sufficient if done correctly.)

  • Are adjustable boom lifts used and have employees been trained on there use? (Toolbox talk is sufficient if done correctly.)

  • Are employees using a personal fall protection system in articulating boom lifts and scissor. (Harness are only required in scissor lifts if the customer requires there use.)

Focus 4 Main Hazards

Caught In Hazards

  • Are guards in place on Tools and Equipment

  • Do pathways have clear access and egress?

  • Will confined spaces be entered? (If the answer is yes complete confined space permit in heath and safety manual.)

Struck By Hazards

  • Are reflective vest worn by all employees?

  • Are materials being stored windsafe?

  • Are areas protective from overhead hazards? (Toe boards or falling object protection where appropriate).

Electrical Harzards

  • Has the company's assured grounding program been implemented?

  • Are weekly GFCI test being conducted?

  • Are electrical danger signs on all electrical rooms and any where an electrical hazard exist?

  • Is there lockout/tagout equipment available on site?

  • Are temporary lights on a dedicated circuit?

  • Are temporary lights properly supported?

  • Are all tools and equipment grounded?

Fall Hazards

  • Are handrails on all stairwells?

  • Are there guardrails or adequate floor covers on all floor openings? (A floor whole is considered any opening greater that 2 inches).

  • Are guardrails on open sided edges, walls, windows, roof edges over 6 feet?

  • Are employees using personal fall arrest systems (PFAS)? (Fall Protection Harness, Lanyard and Anchoring Device).

  • Have employees been trained on the use of PFAS?

  • Are anchorage points capable of supporting 5000 pounds?

  • Have PSAS been inspected?
