Installer audit/check sheet

  • Installers to call the store prior to site arrival to ensure that the store has not closed early due to staff sickness.

  • Mitie work wear and Identification badges are to be worn and displayed at all times whilst on site.

  • Installers to sign in to site log books and undertake all required permit related activities.

  • Installers must log in to the PCS portal and download the store asbestos register and report prior to commencing any work related activities. The client will be monitoring this remotely.

  • Generic & site specific risks/RAM's must be reviewed, prepared and signed off prior to commencement of work.

  • Obtain key's & codes from Ladbrokes staff and/or confirm site lock up process prior to store closure.

  • Site audit to be completed prior to commencement of works. The audit must include 4 photos from each corner of the store along with 4 lux level readings. Lux level readings must be taken from the cash desk area, one area where writing takes place, ​the open area 2m from the front door in a central location, 2m from the cash desk in a central location drawing. ​Please upload lux level readings and photos to this form.

  • All operatives attending site must wear mandatory PPE as set out in the site specific RAM's.

  • Isolate and lock off circuits to be worked on.

  • Face masks to be worn when removing ceiling tiles or accessing ceiling void.

  • Use Gloves when handling tiles and fittings.

  • Undertake the lighting installation as per the agreed specification and drawings. Please note that some fittings may have to be moved due to A/C units, glare on gaming machines/ TV’s or poor light levels.​ Please update the drawings to reflect the lighting installation.

  • Install Emergency lighting as per design.

  • Prioritise the main shop floor lighting installation prior to completing all other remaining works.

  • Lighting circuits to be tested and re-energised upon completion of works. A minor works certificate must be filled out upon completion of works. Please upload photo evidence.

  • Window lighting time clocks to be reset upon completion of works. Time set points are on at 08:00 hrs and​ off at 23.59 hrs.

  • Site audits must be undertaken upon completion of works. Evidence must include 1x as fitted sketch drawing, 4x lux readings & 4 photos taken from the same location as the pre-install audit. Please upload lux level readings, drawing and photos to this form.

  • Installers to vacuum floors and clean down desks/worktops, seating, etc. and leave the store as it was found.

  • All waste must be removed from site and disposed of as per the guidance issued by the Project Manager.

  • Installers must sign out of store log books and complete all permit related activities prior to leaving site.

  • Installers to request store staff member (preferably store manager where possible) signature confirming that they are satisfied with the installation where appropriate.

  • Installers to upload a copy of the Entain (store staff to complete) pre-start & post completion check list. Failure to do so could lead to a return journey and/or charges for claimed store damages.

  • Installers to secure the store and return the keys to the appropriate Ladbrokes employee as per the guidance issued by the Project Manager.

  • The installers store pack must be issued to Mitie within 24 hours. The store pack must include as fitted drawings, work record sheet/engineer's timesheets​, survey/Install record​s, minor works certificates, EML test certificates​, before and After Photos & lux level readings documented via IAuditor.

  • Installers sign off upon completion of works.
