    The Evaluator will be the Superintendent that is tasked with overseeing the operation on the location in which the Lead Operator is working.
    This Score Card is a daily review of the Lead Operator and shall be done within 48 hours of the assessed date. Also, the full review shall be uploaded to the Dropbox with in a week of the Lead Operator leaving for scheduled off time.

Observation Cards (BBS)

  • Has each person working on location tuned in at least on BBS card in the last seven days?

Incidents/ Reports

  • Where there any spills?

  • What is the product, volume and Incident Report number.

  • Were there any injuries?

  • What are the extent of the injuries, Doctor or Hospital, and Incident Report Number?

  • Where there any property damages?

  • Was it third party rentals or EDS equipment and what is the incident report number?

  • Where there any near misses?

  • What is the Incident Report Number?

Daily Paperwork

  • Have all preventative maintenance reports been completed?

  • Has each person on site reviewed and signed a task specific JSA for the current shift?

  • Has the Lead Operator reviewed the DOR with the Company Representative? If no give an explanation.

  • Has a fluid transfer form been completed for each transfer?

Housekeeping

  • Is the bunk/staff house clean and orderly?

  • Is the crew truck clean and orderly?

  • Is each piece of field equipment clean and orderly? MCU, MDU, SMD, DWT, etc.

  • Is the overall condition of the backyard clean and orderly?
