  • Special tools or brushes in place to remove chips, vinyl or other debris from point of operation/pinch point(s)?

  • Powered electrical equipment has an on-off switch.

  • Machinery located so that operator and others tending to machine do not stand in an aisle?

  • Red emergency stop buttons at operator station which will stop machine/motor when suppressed if operating hand transfer mechanical power press or if work station is remotely located from the machine controls.

  • Actuating controls are guarded or located to prevent accidental actuation.

  • Lock out / Tag Out instructions/procedures are documented

MACHINE GUARDING

  • What type of guarding is utilized to eliminate hazards at point of operation, pinch points and rotating parts are adequately guarded (can not reach over, around or under the guard to point of operation or hazard)?

  • Are all machine guards in place and securely attached to equipment

  • Have fixed guards been modified, cut and/or adjusted causing the potential of an injury?

  • Are light curtains or interlocking guards being used as guarding?

  • If light curtains and/or interlocking guards are being used as guarding, what is the minimum stop time distance requirement for all sides of equipment? (Front, Sides, Back)

  • Are two hand palm controls being used?

  • If palm control stations being used for activation, what is the minimum stop time distance requirement for all sides of the equipment? (Front, Sides, Back)

  • Do palm two-hand palm controls ensure that both hands must be held in place throughout the complete operation of a single cycle?

  • Are fixed guards and/or perimeter fencing securely fastened to require tools to remove?

  • Are interlocking and/or light curtains wired in the controls to stay active at all times while equipment power is on?

  • Are die blocks interlocked and located at/on press to be used when performing lockout activities?

  • Is perimeter fencing being used to guard equipment?

  • Is perimeter fencing no more than 6 inches from ground, at least 60 inches high and high enough to prevent objects being thrown out from the highest reach of a robot?

  • If power press, what is the stop time distance for palm stations?

  • If power press, what is the stop time distance for presence sensing devices?

ELECTRICAL SAFETY

  • What make/model of controller is being utilized on this machine

  • Do electrical boxes have three feet or clear space maintained in all directions from any opening or operational sides.

  • Do electrical panel boxes have lids or doors that are in good working condition (not damaged.)

  • All electrical service cords are in good condition and have grounding conductor.

  • Is appropriate voltage labeled on all electrical circuit breakers and panels.

  • Are all circuit breakers clearly labeled as to which direction is "on" and "off".

  • Equipment and processes are wired to eliminate the use of extension cord as permanent wiring.

MAINTENANCE

  • Safety devices or systems required for this equipment are properly installed, adjusted and verified as required based on manufacturer recommendations or OSHA/ANSI/NFPA safety standards

  • If mechanical power press that is or may be hand transfer, is brake monitor installed and anti-repeat logic programmed in controls.

  • Inspection schedule is in place per manufacturers recommendations.

  • All energy sources have been identified and labeled as Lock Out Points.

  • Inspection covers all items recommended by manufacurer.

HOUSEKEEPING

  • Materials, including scrap and debris are contained in a manner that does not create a hazard.

  • The floor of the work area is free of hazardous accumulation of debris, oil and water and other slip/trip hazards.

  • All emergency stop devises on machines (except cables) are red in color.

  • Unobstructed 3-foot aisle maintained between machines.

  • Machine securely anchored to the floor preventing inadvertent tipping/moving.
