Permit To Work (PTW) and supporting documents

  • An approved PTW and properly completed supporting excavation permit/certificate is available at the jobsite (commissioning permits are required for works within commissioned/energized corridors)

  • The lastest services/As Builds/ Cable drawings are attached to the PTW as well as the drawing highlighting the excavation requirements.

  • The supporting JHA and Method statement are available with the PTW & specific to the activity being performed.

  • Daily excavation and trenching checklist has been properly completed and attached to the PTW.

  • AA, RAR & Work Leader has jointly conducted an inspection of the jobsite to validate controls are in place.

  • All work crews have participated in the daily Safety Task Analysis (STA) conducted at jobsite not shelter and associated hazards identified and controls.

  • The permit has been properly revalidated by appropriate persons for the allocated period of work.

  • The Permit Receiver/Supervisor is available at the worksite (or in case proximity) to monitor the safe excavation of the tasks.

Excavation Safety

  • Are excavations or trenches, greater than 1.0 m deep, benched, sloped, shored and signed off by a geotechnical engineer as safe to enter) refer to requirements specified on the PTW.

  • Hard barricade gas been established a minimum of one (1) meter from the edge of the excavation.

  • Suitable warning signage has been installed around the excavation work area.

  • A safe means of access and egress into/out of the excavation has been established.

  • Stock pile/ Spoil are placed further than one (1) meter away from the edge of the excavation.

  • Vibrating equipment or heavy machinery is not permitted within one (1) meter or benched or shored excavations.

  • Hand digging are utilised to expose and safe guard buried services within 1 meter radius (i.e. 1 meter to the left and 1 meter to the right of horizontal plane) from the buried installation.

  • The excavator operator and spotters has the latest buried service drawings readily available and is able to communicate what buried services exist.

  • All personnel trained and competent to perform their assigned tasks (check PTW training, OUIJV induction, Excavator operator competency).

  • Conflicting work activities (SIMOPS) have been identified and risk mitigation measures established.
