Facilities conditions/equipment/toilets/water: clean, safe, secure and tidy?

JSEA & Prestart forms relevant to current activities and signed by all staff ?

TMP current and on site?

Emergency procedure: in place, understood by all staff, displayed?

Hazardous substances: storage, handling, use, labelled, MSDS available?

"Permits (Dig/DoL/Hot works): appropriate, complete in full, sign-off obtained? Evidence of measures to protect utility services are being managed"

Plant: Mobile equipment prestart inspections completed? Operator checklists completed daily? Stationary plant (pumps etc.)

Health and safety information: notice boards, displayed and current?

First aid equipment: complete, available and correct? Cut book in first aid kit?

Fire fighting equipment: secure, easy accessible, and current certification (yellow tag, annual check and 5yr pressure test)?

Electrical equipment: tested and tagged, suitable for task and safe? Equipment register

Lifting equipment: tested and tagged, suitable for task and safe? Equipment register

Power tools tested and tagged,suitable for task and safe? Equipment register, training register

Protective clothing: equipment as required, maintained, available and being worn?

Perimeter Fencing, suitable, controlled?

Excavation support system: Shields & temporary works certified, suitable, safe and secure?

Edge/fall protection in place?

Access and egress: suitable, safe and secure to all areas? Ladders?

Lifting/rigging operations: correct and safe? Dogman adequately trained (unit standard 20875 or equivalent)? Training register

Confined space entry: Permit, trained personnel, Certified Harness, Tripod and gas detector calibrated? Training register

Scaffolding: suitable, design for working platforms, correct set-up, safe & secure?

Are all environmental risks/hazards identified on HSE board?

Does the site have an FCC Site Environmental Folder?

Is the site specific A3 EMP visible?

Are all materials and equipment stored correctly?

Are spill kits available and fully stocked?

Are ACM and coal-tar being managed and disposed of appropriately?

Are stockpiles located appropriately with stormwater diversion pipes?

Are waterways adequately protected by appropriate measures?

Are catchpits adequately protected?

Is sediment tank(s) operating properly. Is there sediment build-up in the tank, and are 2 hour checksheets being completed?

Are sediment/silt fences installed correctly and fit for purpose?

Is the discharge path clean? Note: Discharged Water must be clean at point it enters catch pit

Is overpumping secure from traffic & vandalism? Fenced? Cable tied couplings? Pipe protected?

Is the over pumping occurring consistent with the Wastewater Over pumping Best Practise Guide?

Are trees properly fenced and drip lines free of plant and materials?

Is dust nuisance being managed appropriately?

Are pumps/generators appropriately located - with effective mitigation?

If excavating in Cat 1 or Cat 2, is the Monitoring sheet being completed?

Are all heritage items on site adequately protected?

If working in a High Risk Archaeology area, has a crew been briefed by an Archaeologist?

Is the standpipe CCC approved and been read and serviced in the last 3 months?
