Manual Material Handling

  • Are mechanical devices being used in place of manual handling of material?

  • Are ropes, slings, chains, and chokers in good condition?

  • Have proper storage practices for flammable been observed?

  • Are extinguishers readily available and serviced regularly?

Housekeeping: Slips, Trips, and Falls

  • Are walking and working surfaces clear and free of debris?

  • Are waste and trash containers provided, used?

  • Is there regular removal of waste and trash from the containers?

  • Does each trade clean up after themselves?

  • Is temporary storage of material and supplies done in an organized fashion?

Crane

  • Are cables and sheaves checked?

  • Are slings, hooks, eyelets, chokes inspected?

  • Are load capacities posted in cab?

  • Are power lines at a safe distance?

  • Is the crane properly barricaded around swing radius?

  • Are daily inspections done and available to see?

  • Outriggers fully extended? If no explain why.

  • Has rigging been checked and all bad rigging taken out of service?

  • Are tag lines being used to insure load is under control?

Site conditions

  • Have all FAI employees on site gone though the contractors orientation?

  • Do we have a crane path around building?

Fall protection

  • Are all SRL's in good working order? If not list make and length of SRL's taken out of service

  • What is the fall distance on each floor? Enter distances below

  • Do all the crew members have good harnesses that have been inspected in last 6 months?

  • Dose crew have correct equipment for the application in which they are using?

Welding / Cutting

  • Natural or mechanical ventilation adequate?

  • Are welding leads in good shape and a 10 ' whip installed on stinger end?

  • Are gas cylinders, hoses, regulators, torches, torch tips, and welding machine in good condition?

  • Surrounding area free of flammable a and combustibles?

  • Does torch have flash arresters on bottle side?

  • Do they have HOT WORK permit of required by contractor?

  • Welding hood, gloves, and proper PPE being used by welders?

Structural Steel

  • Employees using the required fall protection equipment?

  • Danger zone beneath the steel erection designated to limit unauthorized employees?

  • Proper erection bolting and bracing procedures followed? Attach bolting method in comment.

  • Ladders, stairways, approved personnel lifts or other safe means or access?

Floors, Wall Openings, Stairways

  • Floor and roof openings guarded by guardrail and toe boards or a secured cover?

  • Open-sided floors/platforms six feet or higher guarded with railing, toe boards or equivalent?

  • Are stairs with four or more risers equipped with standard hand rail protection?

  • Runways four feet or more above ground properly guarded?

  • Anchor posts and framing capable of withstanding 200 lb load in any direction?

Equipment

  • Operators properly trained and authorized?

  • Inspection and maintenance performed on a regular schedule?

  • Moving parts protected by guards?

  • Roll over protection provided as required and with seat belts in forklifts?

  • Are all safety chains on currently and operators tied off to proper points of machine?

Power Tools

  • Are the extension cords all up to standard? ( ground pins and no cuts )

Medical

  • Are first-aid kits available and properly stocked?

  • Are emergency phone numbers posted?

  • Are employees aware of the address of the site and capable of giving directions to emergency crew?

  • Is anyone trained in first-aid and CPR?

  • Does the crew understand the rescue plan if a worker has fallen?

Hazard Communication / Does the program include:

  • A list of hazardous chemicals?

  • Material Safety Data Sheets ( MSDS )

  • Employee training?
