Inspection Item

Means of Access

  • 1. Provision of proper and safe access for vessel boarding and disembarking

  • 2. Ensure all edges of vessel are properly and fully fenced off to prevent person falling into the sea

  • 3. Provide efficient lighting at all means of access and egress

Training

  • 4. Ensure the vessel is securely moored and berthed while performing operation

  • 5. Ensure the mobile equipment/plants are fixed and anchored securely

  • 6. Ensure no works over water under adverse weather condition

Fire Prevention & Emergency Facilities

  • 1. Ensure all flammable substances are properly handled, stored and marked with labels

  • 2. Ensure storage of diesel drums with reasonable distance from any heat source

  • 3. Provide appropriate fire extinguisher on board as per the license and in workable condition

  • 4. Provide and maintain the first aid box is checked and replenished regularly

  • 5. Provision of mobile phone or VHF radio (if necessary) on board for emergency call

  • 6. Rescue boat is available at work location

Personal Protective Equipment

  • 1. Ensure everyone to wear safety helmets, gloves, safety shoes and lifejacket throughout the voyage

  • 2. Ensure all PPEs are in good condition and no expired

  • 3. Ensure all lifejackets are serviced by authorized agent/manufacturer at least once a year

  • 4. Provision of Ear Protection Zone for workers at noisy area and provide workers with ear plugs

  • 5. Provide and ensure workers to wear safety harness at height

  • 6. Ensure additional PPE is provided as per the requirements from risk assessment

Machinery

  • 1. Provide and maintain machine and moving parts with safety guards

  • 2. Maintain insulation to hot exhaust gas pipes

  • 3. Clear annotation and labelling for the control panel and all information clearly marked

  • 4. All drilling machines have been examined by RPE and relevant valid certificates are available on board

  • 5. All drilling rods are stacked in rack and wedged securely and properly

  • 6. The S.W.L. is permanently marked on machines and winches

Lifting Appliances and Lifting Gear

  • 1. Keep all the LALG with valid test certificates and color-coded

  • 2. Ensure all lifting appliances and drilling rig/equipment are operated by qualified operators

  • 3. Ensure all lifting appliances to mark clearly with SWL indication

  • 4. Ensure all shackles and pins securely threaded with safety loop to avoid dislodge

  • 5. Ensure that Form 1 (F&IU) for LA is duly competed weekly

  • 6. Provision of logbook for LALG and signed by Competent person

Hand Tools

  • 1. Provide good housekeeping of hand tools in work

  • 2. Provide regular check and inspection on condition of hand tools

  • 3. Stop and remove any defective hand tool in work

Chemical Waste

  • 1. Provide proper storage/ container for any chemical waste/ material

  • 2. Ensure all chemical wastes/ storage are clearly marked or displayed.

General

  • 1. Provide adequate lighting any time in work and make sure no interference in any course of navigation

  • 2. Ensure that the permit to work system is functioning and valid in work

  • 3. Provide adequate welfare facilities on board such as toilet, drinking water and washing tank

  • 4. Ensure no alcoholic drink and drug be allowed in work

  • 5. Underground utilities and cables are identified and clear off from drill alignment

  • 6. Provision of risk assessment and method statement for the operation
