  • Has the scaffold been constructed and loaded in accordance with the design of a qualified person with a safety factor of 4 to 1?<br>

  • Has the scaffold platform been fully planked with less than 1 inch between planks or does it meet the maximum requirement between plank and the upright?

  • Where the employer can demonstrate the necessity, is the gap between the last plankand the up rights (verticals) less than 91/2 inches when using wood boards, 9 1/4 inch with steel boards, or 19inches with aluminum boards?

  • Is the Vertical distance from the top tie to the top of the scaffold less than the 4 to 1 minimum base dimension?

  • Are ties installed at each end of the scaffold and at horizontal distances not to exceed 30 feet?

  • When eccentric loads (trash chutes, stair towers, weather proof wrap) are imposed have ties been installed to counteract these loads?

  • Are scaffold erected on adequate firm footing, capable of supporting 4 times the intended load without settling?

  • If using standard frame scaffold and the manufacturers built in climbing ladder, was the frame manufactured to be used as a climbing ladder? This is identified by evenly spaced rungs even between the stacked frames, and rungs that are 8" or wider, along with other manufacturers steps taken to keep the ladder a safe use for access.

  • Is direct access from other structures prohibited when that distance is more than 24 inches vertically or 14inches horizontally?

  • Has the movement of rolling scaffold been prohibited? (Unless designed by a Registered Professional Engineer) ROLLING TOWER SCAFFOLD ONLY

  • Do scaffolds meet the proper safety clearances from electrical power lines as set out in 1926.451 (f) (6) ?

  • Are tag lines being used to control loads being hoisted onto or near scaffolds (use of electric hoist or pulley wheel /pulley arm)?

  • If storms or high winds are present or become present has the Competent person been consulted with the plan of action, and has he made all workers aware of the proper protocol. Winds at 25 MPH and exceeding require the additional use of personal fall arrest. All loose material are to be tied down or removed and all debris netting need to be attached with breakaway type straps.

  • Are tools, materials, and debris being removed regularly from the scaffold to ensure that there is no accumulation?

  • Ladders are prohibited from being set up and used on top of scaffold decks, Unless the criteria set out in 1926.451 (f)(15) (i,ii,iii, and iv) that will allow for ladders to be set up on scaffold. Does the scaffold Meet all the requirements set out in the above standards?

  • Have all walk boards been designed or provisions made to prevent platforms from deflecting more than 1/60 of the span of the walk board?

  • Are guard rails and mid rails installed on all open sides and all openings of the platform? On Lend Lease jobs this includes a additional 60" height rail for a total of 3 (top rail, mid rail, bottom rail)?

  • Are guard rails installed at 36 to 45 inches in height and midrails at 50% of that height?

  • When mesh or screen are installed on the scaffold do they extend from the top of the guard rail to the platform?

  • Does the entire guard rail system meet the strength requirements as stated in 1926.451 (G)(4)(vii,viii and xi)?

  • Have fallen object hazards been eliminated according to 1926.451(h)?

  • Have toe boards been installed to prevent falling objects?

  • Fallen object protection is based on height of fall protection material i.e. 2x4x16 = 3 1/2" tall protection

  • Where required due to the material being used and the type of work preformed have screens been installed to protect employees from falling object protection?

  • Is scaffold being inspected before each shift begins work everyday and if multiple shifts in one day then it must be inspected multiple times before each shift, also if there is any accident which affects the scaffold or any changes? the competent erson or qualified person must be called back for a complete reinspection.

  • Signature of the Competent or Qualified person Doing the initial inspection after erection is completed

  • Signature or name of the customer who is to do the daily inspections and is receiving the turn over from CFSS
