Field Service LSOP Observation Checklist

Observation Instructions

  • With the Field Service TM, observe the scheduled job/tasks using the Site Observation Checklist that focuses on the Terex Life Saving Behaviours that will allow you both to later discuss the observation findings

  • Ask questions to understand how the TM meets the LSB focus requirements and where they are being challenged. Enquire why the identified requirements are a challenge so they be escalated and resolved.

  • Refrain from providing feedback until after the task has been completed

Task Safety Preparation - Prior to the TM(s) Performing the Task/Process

  • Has the FS TM completed the Field Service Risk Audit Tool?

  • Do any of the Workplace Safety Conduct Behaviours apply to the observed task/process?

  • Which Life Saving Behaviours apply

  • What Terex Injury Prevention Behaviours apply?

  • What Genie AUS Cardinal Rules apply?

  • What HSE compliance requirements are required for this task

  • Is the work area clear of Slip, Trip and Fall hazards?

  • What Training, Operator License, SOP's or Work Instructions (WI's) requirements apply to the Task/Process being completed?

  • Has the FS TM received the required training and/or hold the required licenses?

  • What PPE is required for the FS TM in order to safely complete this task or process?

  • Are there any weather or environmental hazards?

Task Safety Observation - During the TM(s) Performing the Task/Process

  • Are there any hazards that present an imminent danger to the FS TM that require an immediate Job Pause on the task/process being performed?

  • What were these hazards and risks?

  • Ergonomics - are there any manual handling or ergonomic hazards being created within the task/process?

  • What are these ergonomic hazards?

  • Have site traffic management hazards been identified and controlled?

  • Are there any machine access hazards that may make the job unsafe?

  • Are there any overhead hazards present e.g. construction work, crane operation?

  • Is the FS TM required to work at height and is this being adequately controlled?

  • Is the FS TM using tooling and equipment in a correct and safe manner?

  • Is the FS TM following all Workplace Safety Conduct Behaviours?

  • Is the FS TM handling chemicals safely and protecting themselves from exposure?

  • Is the FS TM wearing the required PPE correctly?

  • Task/Process Observation Notes - detail all the observed safe and unsafe behaviours

  • Is the FS TM working in accordance to any required SOP or WI?

Task Safety Observation Conclusion - After the TM(s) have completed the Task/Process

  • Feedback to the FS TM(s) - What were the safe behaviours identified during the Task Safety Observation?

  • Feedback to the FS TM(s) - What were the unsafe behaviours identified during the Task Safety Observation?

  • What Actions or Countermeasures are required to address or rectify the the unsafe behaviours? e.g. training, tooling and equipment, procedure/processes
