Current SWMS, employee declaration signed by all personnel, Pre start safety committee meeting conducted ?

Any changes to the work scope/plan have been communicated to workers ?

Correct PPE available and used for the task ex. Eye, hearing, dust, chemical, WAH, and hot works ?

Communications plan in place ?

Restricted access delineation bunds, barricading, and signage in place ?

Machinery/plant and equipment Pre starts are complete, damage, faults reported by workers ? Any damage not reported ?

Crushing plant inspection complete ?

Safety guards are in place on all Plant, tools and equipment ?

Housekeeping ( work area clear of trip hazards, equipment stored correctly ) ?

Fire extinguishers in place and in date on all plant and equipment ?

Fuels, oils are stored correctly and secured for transport ?

Work area is safe for operating ( quarry face is safe for operating ex. No over hanging rocks above load face area ) ?

All excavations are safe ( benching, battering, sharp drop offs barricaded/bollards/sign posted ) ?

Workers with First aid on site ?

Weather conditions ?

Remote workers communication plan in place ?

Oil spills cleaned up, Hydrocarbon waste stored correctly ?

Dieback, are vehicles free from mud/dirt when entering or leaving site ?

Are controls for spread of weeds or other diseases in place ?
