General

  • Has a Pre-Start Briefing been conducted with workers and are workers aware of the specific hazards/risks to the specific work activity and tasks to be undertaken?

Plant

  • All operational plant on site has a registered Plant Approved sticker?

  • Pre-start checklist completed?

  • Operators have been deemed competent or are suitably supervised?

  • Is the plant correctly parked? (Eg. attachments, blades lowered, not idling unattended)

  • Is the plant free from damage and in good condition? (No missing or damaged guards, no obvious leaks, tyres ok etc.)

  • Has a plant risk assessment been performed and is it available?

Scaffolding

  • Scaffolding has been erected by appropriately licensed workers?

  • Scaf tags have been completed for each scaffold structure within 30 days by a competent person?

  • Safe access to and egress from scaffold has been provided during erection, use and dismantling?

Formwork

  • Levelled base plates / sole boards are provided under props and standards? No wash outs or uneven bedding?

  • Edge protection is provided where required?

  • Ladders used for access are appropriately secured?

  • Perimeter containment screening is installed with no gaps of more than 25mm?

  • Reo bars are adequately capped or do not present a risk of injury?

Cranes and Lifting Operations

  • Crane operators have relevant high risk work licenses - C2 (up to 20t), C6 (up to 60t), C1 (up to 100t), C0 (over 100t)

  • Vehicle Loading Crane (capacity of 10 metre tonnes or more) operators have Vehicle Loading Crane HRW license or C2, C6, C1 or C0? <br>Metre tonne = Lift Capacity x Radius

  • Crane operators, doggers, riggers have been verified as competent?

  • Lift Plans are in place for all major lifts (Heavy lifts >50t, Bridge beams, Multiple crane lifts, Workbox lifting persons, Vicinity of power lines, On or above water, Unusual or odd shaped items) and the lift plan is included in the SWMS?

  • Workers involved have been trained in the Lift Plan?

  • Lifting equipment has been inspected daily by operators prior to use?

  • Lifting equipment has been inspected quarterly by a competent person (eg. dogger, rigger) with test tags attached?

Electrical

  • Electrical work is only performed by a qualified electrician?

  • Work conducted near electrical power lines is done so under an Electrical Vicinity Permit, where required?

  • Construction power is protected by an RCD?

  • Electrical equipment is tested and tagged by a competent person within the required period?

Excavations and Trenching

  • All excavations or penetrations have an authorised excavation permit completed prior to commencement of excavation work?

  • Benching, battering or shoring has been used to control the risk of ground collapse in excavations deeper than 1.5m?

  • Adequate controls are in place to prevent falls into excavations and falling objects?

Working on or Near the Road Corridor

  • All relevant workers have signed on to a SWMS for this work?

  • Traffic control is in place where required?

  • Do all workers with responsibility for traffic control, hold the relevant training and qualifications?

  • Traffic controllers have access to the current approved TGS?

  • The traffic management set up is compliant to the approved TGS?

  • Does the TC have an adequate escape route?

  • Are emergency contact numbers posted where temporary traffic controls operations will remain in place after-hours?

Working at Heights

  • Permit is in place for Work at Heights?

  • All relevant workers are trained and VOC is reasonably available?

  • Harnesses and other WaH equipment is checked, tagged and in date?

  • Anchorage is suitable, 100% hook up is used?

Other Permits to Work

  • Permits are in place for Concrete Cutting and Drilling?

  • Permits are in place for Hot Works?

  • Permits are in place for Work in QR Rail Corridor?

  • Permit is in place for working near live electrical equipment?

Amenities

  • Are toilets and hand washing facilities available on site or within a reasonable distance? (Eg. a short drive & vehicle available)

  • Is drinking water available?

  • Are suitable facilities available for meal breaks?

First Aid

  • Are there sufficient 1st aid kits available?

  • Is there qualified First Aid personnel on site?

PPE

  • Is PPE worn by workers compliant to site requirements?

Hazardous Substances

  • Have all hazardous substances been identified?

  • Are all hazardous substances correctly stored?

  • Are containers clearly marked / labelled correctly?

  • Is there appropriate ventilation for use and storage?

  • Are Safety Data Sheets (SDS) available to workers and in date?

  • Is a spill kit available?
