  • Do scaffolds have a (4:1) base to height ratio

  • Are scaffolds securely anchored at least every 30' of length and 25' in height

  • Is there an access ladder, steps or equivalent safe access to scaffold

  • Is the scaffold within its weight capacity

Environmental & chemical safety/controls

  • Hazard communication explained to personnel

  • Safety Data Sheets (SDS) available for all chemicals
