  • Are weekly safety meetings held with all coworkers and subcontractors?

  • Have all coworkers been issued safety equipment including hard hats, safety glasses, and gloves?

Hazardous Substance Communication

Material Handling and Storage

  • Are compressed gas cylinders properly stored?

  • Are motors on equipment being refueled shut off?

  • Is smoking prohibited in the vicinity of fueling operations?

  • Are materials, stored inside buildings under construction, placed more than 6 feet any side or opening, or more than 10 feet from exterior walls do not extend above the top of the material stored?

  • Are coworkers using gloves when handling material?

  • Is rigging equipment inspected each shift prior to use, and removed from immediate working area when not in use?

Electrical

  • Are GFCIs in use?

  • Are ground fault circuit interrupters installed on each temporary 15 or 20 amp 120 V AC circuits at locations where construction, demolition, modification, alterations or excavations are being performed?

  • Is temporary lighting used and proper?

  • Are lock Out / Tag Out procedures in place and used?

  • Are temporary cords properly coded?

  • Are temporary cords properly used and maintained?

  • Do all boxes, panels have covers and are all circuits labeled?

  • Are all electrical tools and cords checked once a month by a competent person and are electrical tools and cords color coded to show compliance with the 30 day check?

Tools and Equipment

  • Are all guards in place and in good repair?

  • Is equipment being used properly without any unsafe or incompatible attachments?

  • Are all operators qualified?

  • Is equipment inspected on a regular basis?

  • Are goggles and full face shields required for such operations as grinding, jack hammering, or utilizing compressed air?

  • Are portable fans provided with full guards or screens having openings of 1/2 inch or less?

  • Are coworkers who operate powder actuated tools, trained in their use, and carry a valid operators card?

  • Is each powder actuated tool stored in its own container and locked when not in use?

  • Do all handheld tools have appropriate "on-off" controls including momentary contact controls when required?

Ladders and Step Ladders

  • Are all ladders in good condition?

  • Are all ladders of the proper duty rating?

  • When used, are ladders being positioned properly?

Fall Protection

  • Are all rebar and pipe or other impalement hazards capped or protected?

Equipment

  • Are platforms kept clean and free of debris?

  • Are all operators trained in equipment safe use and for possible hazards?

  • Is all equipment inspected daily and documented on appropriate form?

  • Are harnesses being used?

  • Has the lanyard been connected to a proper tie off point?

General Site Issues

  • Is suitable storage for waste provided on site?

  • Is lighting adequate?

  • Is storage adequate and safe?

  • Is the site safety signage adequate?

  • Is overhead protection provided at entrances where overhead work is ongoing?

  • Are aisles kept clean, is scrap removed?

  • Is there an adequate supply of water, water jugs, cups, and trash containers?

  • Are toilet facilities adequate?

  • Is public protection adequate including warning signs?

  • Are vehicles accessing the site safely?

  • Are all deliveries occurring during prescribed times?

Welding and Cutting

  • Are all welders wearing proper welding PPE?

  • Are cylinder valves closed when empty or when work is finished?

  • Are unused cylinders secured in an upright position when not in use?

  • Are all torches inspected at the beginning of each shift?

  • Are welding cables inspected at the beginning of each shift?

  • Are welding cables free from damage, repair, and splices for a minimum distance of 10 feet, from the cable end to which the electrode is connected? ( except that cables with standard insulated connectors or with splices whose insulating quality is equal to that of the cable are permitted.) 29 CFR 1926.351 (b) (2)

  • Are flame arrestors used on hookups?

  • Is there a fire watch assigned for welding or cutting when a serious fire might develop?

  • Is a suitable fire extinguisher available at each welding or cutting operation?

  • Are noncombustible or flameproof screen used for shielding other coworkers, working in the direct vicinity, from the direct rays of the arc?
