Epidural PEDIATRICS Policy GEN112 Appendix A

  • Pt/family teaching is present in the medical record.

  • All patients must be on continuous pulse oximetry for at least 24 hours after epidural analgesia is initiated.

  • Upon initiation or after clinician bolus, or increase in continuous infusion rate, the RN shall monitor the following every 30 minutes X 2, then every 4 hours and PRN: pain rating, sedation level, O2 sat, respiratory rate, heart rate, blood pressure, motor/sensory levels

  • Dressing site will be assessed once a shift

  • Signs of toxicity with local anesthetics will be assessed every 4 hours (confusion, metallic taste, numbness about the mouth, irregular heart beat)

  • 2 RNs must independently validate the drug, dose, therapy qualifier, and pump settings with epidural order and document on the pain flowsheet/MAR per unit standard at the following times: initial set up, any programming changes, when hanging a new bag, each tubing change, and when accepting a patient transferred from another area who is receiving epidural analgesia.

  • RN will validate correct drug, dose, line to catheter connection, and pump setting against the prescriber order, epidural tubing, and catheter are labeled with yellow tape on each side of the connection and document on the pain flowsheet: at the beginning of each shift, prior to and upon return from a procedure, diagnostic, or consultative area, and when transferring the patient to another unit.

  • Floor stock insulin is limited to aspart insulin in refrigerators on adult inpatient nursing units and either aspart or regular insulin on pediatric units. Other forms of insulin will be labeled patient specific or located within a Pyxis machine. <br>Only non-patient specific aspart insulin stored in inpatient refrigerators-all others in patient cassettes. NPH OK only for outpatient refrigerators.
