EXCAVATION

  • Underground utilities located (Once Call completed and lines located)

  • Is there an excavation more than 4ft?

  • Is there an excavation more than 20ft?

  • Competent Person on site and inspections documented

  • Proper sloping, shielding or shoring as required<br> (Type C - 1.5/1, Type B - 1/1, Type A - .5/1)

  • Access and egress means provided<br> (no further than 20 feet from any employee in the excavation))

  • Spoil set back at least 2 feet

LADDERS

  • Ladders in good condition<br> (stickers, rungs, feet, spreaders, rivets, fiberglass...)

SCAFFOLDING

  • All scaffolds plumb and level

  • Are adequate sills and base plates being used

  • Scaffold Tags present and completed

  • Are wooden scaffold planks installed past their end supports no less than 6 inches and no more than 12 inches

  • Are guardrails including top and mid rail in place

  • To protect from falling objects, are toe boards or area barricaded below

  • Is fall protection being used in areas where required

  • Is there a scaffold tag and has it been inspected

  • Is their a safe access ladder, steps or equivalent means for safe access to the scaffold<br> (Step no greater than 24")

  • Is the scaffold rated for the proper weight capacity

FIRE PREVENTION

  • Are hot work permits in order

  • Fire extinguishers immediately available and inspection tags current<br> (Annually certified and monthly inspected)

  • Is a fire watch monitoring the area

  • Safety Fuel Cans being used with fire extinguishers at proper distance

  • Compressed gas cylinders secured- upright- protected from damage

PPE

  • Hard hats worn

  • Gloves being worn and adequate for the work activity

  • Face shields being used where required

  • Welding hoods being worn in conjunction with safety glasses

  • Eye protection being worn and adequate for the work activity

  • Respirators and masks utilized, when needed

  • High visible clothing in use, when needed

  • Safety harness and lanyards utilized with adequate anchorage points
