  • Laboratory

Laboratory entrances

  • What is the laboratory room number / name?

  • Are there appropriate hazard warning signs at the entrances?

  • Are the signs clearly visible?

  • Are unauthorised persons prevented from gaining access to the laser controlled area?

  • Are precautions in place to safeguard visitors entering the laser area?

Class 3 and Class 4 Laser Systems

  • Are the laser systems appropriately and clearly labelled?

  • Are beam paths enclosed as much as is reasonably practicable?

  • Are beam path components that generate errant beams, locally enclosed?

  • Are all beam paths properly terminated?

  • Do all unprotected open horizontal laser beams lie above or below normal eye level?

  • Are all lasers and optical components on the beam line, securely mounted?

  • Are walkways free from laser beam path crossing?

  • Are all upwardly directed beams, shielded to prevent human exposure?

  • Is a safe method of beam alignment provided?

  • Are interlocks or shutters fitted to the laser?

  • Are the interlocks or shutters tested?

  • Is there key activation for the laser system, with the key kept in a safe place?

  • Are emergency Snatch Cards available?

 The Laser Controlled Area

  • Are the lasers and beam paths kept under the control of competent persons?

  • Are all shiny surfaces (including jewellery) prohibited within the Laser Controlled Area?

  • Are there beam paths normally open in this area?

  • Does the Laser Controlled Area present a robust physical boundary to isolate the laser radiation?

  • Is the boundary (including windows) opaque at the laser wavelengths and without gaps?

  • Is the laser hazard prevented from extending beyond the Laser Controlled Area if a door is opened?

  • Are the laser hazards from separate laser experiments within the area isolated?

  • Are independent non-laser activities prohibited within the area during laser use?

  • Are temporary restrictions imposed for servicing and other non-routine activities within the area?

  • Are the Laser Local Rules available within the area?

  • Is the work area clean and tidy?

Protective Equipment

  • Is laser safety eyewear provided?

  • Does the laser safety eyewear provide sufficient protection for each accessible hazardous laser wavelength (including wavelengths that could be generated by non-linear effects)?

  • Is the laser eyewear properly stored and maintained in good condition?

  • Does the eyewear clearly identify the laser or area with which it is suitable for?

  • Are lighting levels appropriate for the visual transmission of the eyewear?

  • Are gloves, lab-coats, over-shoes and/or masks provided where needed?

Non Beam Hazards

  • Is there a Fire hazard (ie with Class 4 laser beams)?

  • Does the laser generate a fume hazard?

  • Is there an Electrical hazard?

  • Is there an Explosion hazard?

  • Is there a high-pressure gas hazard?

  • Are there trip hazards and sharp corners?

  • Are there any other non-beam hazards?

Documents

General Documents & Records

  • Have all persons at risk of exposure to the laser radiation, received adequate laser safety training and instruction?

  • Has the LPS attended a laser safety course?

  • Has LPS been appointed in writing?

  • Is the latest Laser Safety Management Procedure present?

  • Are the Risk Assessments present?
