Safety

Personal Protective Equipment (PPE)

  • Do you have required PPE? (Gloves, boots, hard hats, eye/face protection where required)

  • Is hearing protection available for personnel that may be exposed to noisy conditions?

  • Is respiratory protection available to personnel and being used when conditions require them?

  • Are safety harnesses, lifelines, and shock absorbing lanyards available and being used?

  • A s personnel using gloves when handling sharp or rough material?

  • Where required, are rubber gloves with protectors-insulators being used?

Fire Protection and Prevention

  • Are evacuation routes posted and clearly marked?

  • Are extinguishers readily available and regularly serviced?

  • Are hydrants clear and accessible for fire department personnel?

  • Have gas cylinders been chained upright with valve caps securely fastened?

Manual Material Handling

  • Are mechanical devices being used in place of manual handling of material?

  • Are ropes, slings, chains, hooks, cables, and chokers in good condition?

  • Have materials been properly staged to minimize lifting and carrying?

  • Has rigging equipment been inspected and found to be in good condition?

  • Is the handling of bagged material limited to 50lbs or less?

  • Are lifting handles being used when a single worker is carrying sheeted material?

Electrical

  • Are switches, panels, and devices that are energized marked and guarded?

  • Lockout devices are available/used in circuits that could become energized during work?

  • Are all temporary circuits properly guarded and grounded?

  • Are extension cords in continuous length without splices?

  • A s GFCI's and/or Assured Equipment Grounding Conductor Program Being used?

  • If temporary lighting is provided, are bulbs protected against accidental breakage?

  • Are working surfaces clear of cords so not to create a tripping hazard?

  • Are there a sufficient amount of outlets on site?

  • Are there any visual signs of outlet overloading?

Hazard Communications

  • Material Safety Data Sheets (MSDS) present or available?

  • Have employees been trained on any materials specific hazards?

  • Have you informed other contractors working in the area of any safety requirements of constraints?

  • Have you posted and required notifications for use of restricted products or area dangers?

Excavation/Trenching

  • Have utility companies been notified of proposed excavation work?

  • Are tools, equipment, and shoring materials readily available prior to job start up?

  • Are overhead utility lines noted and precautions taken to avoid contact with equipment?

  • Is the spoil pile at least two feet from the edge of the excavation?

  • Is the excavation inspected daily or more frequently when conditions could affect the soil?

  • If needed, are barricades, stop logs, properly placed?

  • Hs soil classification been made by a competent person?

  • Are excavations five feet or deeper correct sloped, benched, shored, or is a trench box used?

  • Is a ladder or other means of egress provided in trenches or excavations six feet or deeper?

  • Wen ladders are used, do the extend three feet above the surface and are they secured?

  • Are shoring and shielding systems inspected by a competent person?

  • Is the trench back filled as soon as work is completed?

Barricading

  • Are floor openings planked and secured or barricaded?

  • Are direction signs used to inform the public of upcoming constriction work?

  • Is the sidewalk protection effective?

  • Is a flag person provided to direct traffic when needed?

  • Has that person been trained oh how to direct traffic and the public?

  • Are open excavation, road drop offs, manholes, and uneven surfaces barricaded?

Ladders

  • Is the proper ladder for the job being used?

  • Ae ladders in good condition?

  • Are there safety shoes/cleats at the bottom of the ladder if needed?

  • Are non conductive ladders available for use near live wiring?

  • Are ladders tied off at the top or otherwise secured?

  • Do side rails extend at least 36 inches above top of landing?

  • Are the rungs or cleats uniformly spaced and 10-14 inches apart?

  • Are setup ladders fully open when in use?

Scaffolding

  • Are scaffold components visibly free of any physical damage?

  • Is scaffold properly erected with all pins and braces in place and properly locked?

  • Are rolling scaffolds equipped with locking wheels?

  • Are wheels locked when scaffold is in use?

  • Is scaffold erected on a firm and substantial surface?

  • Is planking of a scaffold grade?

  • Is planking in good condition and properly installed?

  • Are toe board and guardrails in place in scaffolds over 10 feet?

  • Are workers on scaffolding protected from falling objects if an overhead hazard exists?

  • Is a ladder provided for access to scaffold work platform?

Medical

  • Are first aid kits available and properly stocked?

  • Are all emergency phone numbers posted?

  • Ae employees aware of the address of the site and capable of giving directions to an emergency crew?

Hoists Cranes and Derricks

  • Will this job require any hoists, cranes, derricks, or other cable lifting equipment?

  • Please note: KNS Personnel are NOT authorized to operate hoist machinery, however the safety of your subcontract is our responsibility. Please ensure with the operator or supervisor that the following items have been inspected:

  • Are cables and sheaves checked?

  • Are slings, hooks, eyelets, and chokes inspected?

  • Are load capacities posted in the cab or on the equipment?

  • Are power lines at a safe distance?

  • Are there proper barricades around the equipment swing radius and under the load to be lifted?

  • Are the inspection logs with the equipment?

Floors, Wall Openings, and Stairways

  • Are floor and roof openings guarded by guardrails and toe boards or a secured cover?

  • Are open sided floors/platforms six feet or higher guarded with railings, toe boards, or equivalent?

  • Are stairs with four or more risers equipped with standard handrail protection?

  • Are runways four or more feet off the ground properly guarded?

  • Are anchor posts and framing capable of withstanding 200lb load in any direction?

Structural Steel

  • Are employees using proper fall protection equipment?

  • Is a company approved fall protection program in place?

  • Is the danger zone beneath the steel erection designated to limit unauthorized people?

  • Is hoisting equipment and accessories inspected as required?

  • Are tag lines used to control the load?

  • Are ladders, stairways, approved personnel lifts or other safe means of access provided to work area?

Aerial Lifts

  • Are employees using aerial lifts trained and authorized?

  • Are the manufacturers operation and safety rules obeyed?

  • Has the unit safety been inspected and all controls tested prior to each days use?

  • Is the unit positioned on solid, level ground?

  • Are the boom and basket loads within the manufacturers specifications?

  • Is everyone in the lift basket standing firmly on the floor, and wearing fall prevention or protection equipment?

  • Are brakes set and outriggers and wheel chicks positioned as required?
