HOUSEKEEPING

  • Primary walkways and egress points are well organized and free of trip hazards?

FIRE PROTECTION

  • Fire extinguishers are in good condition, inspected within the past 30 days, AND have a documented annual inspection tag?<br><br><br>

  • Are "NO SMOKING" signs posted where applicable (i.e. Flammable Storage Tank, Site Entrance etc.)?<br><br><br>

  • Are Flammable Liquids stored in approved containers? Gas cans are metal "Safety Cans" and have Spark Arrestor Screens in place?<br><br>

  • Has a Hot Work Permit been completed and approved? Is a fire watch present for all areas effected, including lower levels and adjacent rooms if necessary? Have all flammables & combustibles been moved away from welding, cutting, and grinding operations?

ELECTRICAL

  • Is temp lighting in place with adequate lighting in stairways/corridors and power supply stations/turtles have been inspected within the past 30 days?<br><br><br><br><br>

  • Is an Assured Equipment Grounding Conductor Program (AEGCP) in place or all electrical cords protected by GFCI's?

  • Power cords are in good condition and inspected daily?

  • Power Lines are insulated and hazard is communicated to field employees when applicable?

  • Employees are properly trained in Lockout/Tagout, a written procedure(s) is in place with individual locks/tags available to affected employees? Training certificates are on file?

TRENCHING/EXCAVATING

  • Are spoil piles at least 2' back from the leading edge of excavations and trenches?

  • Do Excavations and Trenches greater than 4' deep have proper protective barricading, or proper angle of repose based on soil classification?

  • Has ladder access or other means of safe egress been established within 25ft of ALL workers?

  • Have adequate barricades, and postings been established to prevent vehicular traffic near the excavation/trench and allow for safe pedestrian travel?

LADDER SAFETY

  • Are ladders in adequate condition? Ladders extend at least 3ft over working platform AND secured at the. Ladder is on stable ground and set up at 1:4 pitch? Base of ladder is free of debris and other hazards?

  • Ladders are being used properly? Step ladder Spreader bars locked in open position and not stood up against walls while in use? Ladders are chained/secured when not in use and stored away from primary walkways?

STAIRS/WALKING SURFACES

  • Breaks in elevation >/= 19" have steps in place, been back filled or a ramp has been installed?<br>

  • Metal pan stairs have been temporarily filled or Controlled Access has been established?

SCAFFOLDS

  • Has a documented inspection been conducted each day and tagged with a STOP/GO tag?

  • Has the scaffold been erected on a firm and substantial surface? Are adequate mud sills used when necessary? Note: Base plates or (Casters for rolling scaffold) must always be used.

  • Is a Competent Person supervising the erection/dismantling of all scaffolding?

  • Has adequate access been established to the working level of the scaffold?

  • Is the scaffolding tied in when necessary with adequate guys, ties or wires? Has this been confirmed with Competent Person?

  • Are the wheels on rolling scaffold locked when in use? Is a diagonal brace in place on all rolling scaffold to enhance the structural integrity of the scaffold?

HAZARD COMMUNICATION

  • Are containers properly labeled following the Globally Harmonized System (GHS) or HazCom 2012 standards.

  • Are Safety Data Sheets readily available to all site employees ? It is their "Right To Know"!

PERSONAL PROTECTIVE EQUIPMENT

  • Are ALL employees utilizing Hard Hats, Safety Glasses (With Side Shields), and work boots at all times. Are face shields utilized for all welding, cutting and grinding operations?

  • Are Class 2 reflective vests used per project requirements and/or when working around heavy equipment?

  • Is hearing protection provided by employers and utilized when necessary?

  • Are proper respirators or dust masks utilized when necessary? Have the employees been trained, fit tested, and medically cleared to use respiratory protection?

  • Are employees utilizing proper Fall Protection (i.e. Harness, Lanyard, SRL etc.) when exposed to falls >/= 6ft?

MACHINE GUARDING/TOOLS

  • All mechanical equipment and tools have proper guards in place and are used per manufacturer requirements/as designed?

  • Training has been provided to those who operate a Powder Actuated Tools?

  • Electrical equipment is correctly grounded with 3-pronged plugs or UL Double-Insulated approved?

FALL PROTECTION

  • Are proper guardrail systems in place when exposed to 6' or greater fall potential? Top rail/parapet with a minimum height of 42" (+/- 3") midrail and toe board when applicable. When using wire rope, is it flagged every 6' and set up so it won't sag below 39" with 200 lbs of downward force?

  • Do all floor penetrations greater than 2" located above a lower level have a secured COVER in place with the word "HOLE" painted on it? Can it withstand at least TWICE the maximum intended load?

  • Are tie off points (rated for 5000lbs.) available and utilized properly?

  • Have all applicable trades installed warning lines/barriers 15' back from the leading edge with flagging every 6ft? Note: Only roofers, precast and iron workers can use 6' set back warning lines!

  • Has a Fall Protection Plan with associated training documentation been submitted to KA?<br>

  • Is rebar protection/caps in place to prevent impalement hazards?

MATERIAL HANDLING

  • Are seat belts utilized on all equipment?

  • Are materials safely stacked and well organized?

  • Is all Rigging equipment in good condition, used properly and protected from damage (i.e. softners/proper storage)?

  • Have riggers and signalers been through appropriate training?

  • Has an annual inspection been conducted on each aerial lift? Are daily, documented inspections conducted?

ENVIRONMENTAL

  • Are Carbon Monoxide (CO) detectors available, utilized when necessary, and calibrated?

  • Is Nitrogen Dioxide (NO2) monitoring conducted when necessary?

  • Has all Lead, Asbestos, and other contaminants been removed? If not, are qualified abatement contractors that utilize proper disposal methods utilized? Applicable Training Certificates on file? Are the hazardous material reports on file prior to starting?

  • Have all Confined spaces been monitored for air quality prior to entry with all OSHA requirements followed?

  • Is a wet process, HEPA vacuum or other controls in place to minimize Silica exposure?

PUBLIC SAFETY

  • Are all roadways and sidewalks kept in a safe condition and properly demarcated to protect the public?

  • Is KA Signage in place at all entry points to limit unauthorized access to the site?

  • Is overhead protection in place where necessary?

CRANE SAFETY

  • Is a current Certified Crane Operator (CCO) card on file AND on the operator's personal body?

  • Is the swing radius of the rotating superstructure of the crane barricaded in such a manner to prevent an employee from being struck or crushed by the crane?

  • Have daily, monthly AND annual inspection been conducted and on file?

  • Is the crane set-up on stable ground with outriggers and secondary pads/cribbing used to safely distribute the load and reduce soil pressure?

  • Are all parts of the crane/ rigging a safe distance away from all power lines based on voltage?

OTHER HAZARDS

  • Have all other hazards not listed above been identified and corrected?
