Are all site signs in place?
Please ensure a sign is erected with in 24 hours of this report.
Are temp, fencing and gates in place, adequate and in good condition? Take a photo from the front
A. Re stand fence and tie off panel ends with tie wire.
B. call up a re stand from the supplier.
Have you called for the cage to be emptied?
Are toilets clean and paper stocked etc?
Look for a service sticker. It should be serviced and stocked every 4 weeks
If you require another clean call the supplier.
Are traffic controls adequate?
Have all hazards to the public been eliminated? Ie broken pits, broken footpath bays, risk of trips or slips, protruding debrie, other.
1. Try to remove the immediate risk.
2. Barricade the hazard
3. Arrange a bobcat or other to level out and spread crushed rock if necessary. In relation to broken pits call power core on 131200 to replace the lid
List the contractors currently working on site?
Can the contractor show you a signed thorough SWMS, covering the task he is currently performing?
Can the contractor show you a stocked first aid kit?
Is all relevant PPE being used by the contractors ? Earmuffs, eye wear, sunscreen etc
Is the PPE being used sufficient and in good condition?
Are contractors wearing steel capped shoes?
Are all work areas clean and tidy?
Are all access areas clear of debris and materials?
Are barriers and bunting in place if required?
Are there any dangerous protrusions that could cause injury?
A: either remove the danger
B: Put protection over it, by way of physical barriers,caps,(ensure they will not fall off)
C: If you are unsure speak to the construction manager for advice immediately
WARNING : Never leave a dangerous situation unattended.
Are sharp objects capped (I.e. star pickets, reo etc)?
Are all timber set out pegs removed?
Are all voids protected or covered over?
Are all drop downs greater than 190 mm protected or temporary timber steps installed?
Take a photo,
Are all temp. hand rails in place and secure to withstand 100 kg impact?
Are leads raised where practicable?
Have you raised all leads obstructing walkways?
Are electrical leads and tools tested, tagged and in good condition?
Are all electrical switchboards tested and tagged monthly?
Is the scaffold on secure footing?
Is the scaffold complete and uncompromised by others, such as missing hand rails, ties, clamps or kick boards?
Are there enough ties? ( no wobble )
Does the scaffold give complete access to the required scope of works?
Is the scaffold clean from previous trades? ( ie Bricky mud)
Are the platforms free of debrie, tripping hazards, overloading, materials prone to being lifted by wind?
Has access to balconies been restricted by way of deadlock or hand rails?
Is the stair void/light wells protected even under construction?
Have you eliminated all risk of falls from height?
WARNING : Speak to the construction manager and discuss a solution. DO NOT LEAVE THE SITE WITH AN EXPOSED VOID THAT MAY CAUSE INJURY UNDER ANY CIRCUMSTANCE.
Are all safety guards on and in good condition.ie. grinders, circular saws, drop saws, cement mixers, brick saws etc
Do all lights work on machinery,i.e. reversing beepers, hazard lights etc
Are barriers or bunting erected where required?
Are trenches barricaded off?
Other issues/control measures put in place
Warning issued to:
- No tools tagged
- Not wearing steel capped shoes
- Not covering star pickets with yellow caps
- not wearing PPE
- voids not protected
- verbal warning and instruction o rectify.
